SPECIAL TEAM REVIEW OF THE PERFORMANCE ENHANCEMENT PROGRAM (PEP) CHARTERED BY THE MILLSTONT ASSESSMENT PANEL (MAP)

This letter transmits the results of a special review conducted by a team of five NRC inspectors and reviewers to evaluate the acceptability of the Northeast Utilities Performance Enhancement Program (PEP). This review was conducted pursuant to directions from the Millstone Assessment Panel (MAP). The review was principally conducted during the periods from September 21-25,1992, October 26-30,1992 and November 30 -

December 2,1992, although there has been a cominuing dialogue with your staff regarding PEP implementation. The report details the background of the development of the PEP, the basis for the NRC's review of the program, the activities of the NRC PEP Review Team, the findings of that team and its conclusions. The MAP concurs with their conclusions and intends to pursue the followup actions recommended in report Section V.B.

The NRC PEP Review Team found that the PEP represents a comprehensive program for addressing your performance deficiencies at Millstone Station and the Berlin corporate office, and for enhancing performance at the Haddam Neck site. The program appeared to have management commitment and adequate resources for implementation. Furthermore, the program was progressing on or near its original implementation schedule. However, the verification and validation (V&V) process for the PEP was not yet fully developed, nor were examples of completed V&V activities available for review.

Even though the verification and validation portion of the PEP effort was still in a preliminary stage, the team found that it did contain provisions for an independent validation effort and for appropriate management oversight and coordination. The team noted that the development and use of representative, challenging PEP Action Plan validation criteria is a key for establishing management's expected level of performzace improvement in each area and ensuring that this level of improvement is obtained and sustained.

The NRC has determined that the scope of the PEP is acceptable and concluded that, with a strong and highly visible management commitment to the PEP, the PEP with proper implementation and followup (along with other ongoing initiatives) represents a reasonable approach for addressing the areas of NRC concern as summarized for NU by the NRC in our letter of August 17,1992 (Attachment 2 to the enclosed report). Therefore, the NRC considers the PEP acceptable contingent upon NU submitting to the NRC the details of the PEP V&V process after it is more fully developed and implemented and the NRC finding it acceptable. In addition, the NRC considers it important that you continue to evaluate operating experience at Millstone and Haddam Neck (e.g., the July 6,1992, LRP event at Millstone Unit 2 or the October 1992 requalification failures at Millstone Unit 1) during PEP implementation to determine whether experience or events indicate the need for adjustments to the PEP.

We note that recent NRC inspections have raised some concerns with the implementation of short-term corrective actions as well as continued findings by your Quality Services Department of pmcedure compliance problems. We will continue to closely monitor your performance in these areas, as well as review any future adjustments to the PEP which NU determines are warranted based on performance and/or other reasons.

I encourage you to review the report in detail and to share the findings with the appropriate members of your staff. Should you have any questions or comments regarding the team's findings or the MAP's conclusions, please contact either Richard Barkley or James Wiggins of my staff. The NRC will continue to maintain a current dialogue with your staff regarding the open areas of the PEP review. Upon completion of NRC review of these areas, the NRC will inform you of our final determination regarding acceptability of the PEP.
