SUBJECT:

NRC INSPECTION REPORT 50-482/93-08 (NOTICE OF VIOLATION)

This letter refers to the inspection conducted by Mr G. A. Pick and the

inspectors identified in the attached report for the period of March 28

through May 8, 1993. The inspection included a review of activities

authorized for your Wolf Creek Generating Station.

At the conclusion of the

inspection, the findings were discussed with you and those members of your

staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examination of procedures

and representative records, interviews with personnel, and observations of

activities in progress.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

The first violation resulted from a late surveillance

test.

We are concerned with this example because of the ineffective

implementation of corrective actions developed in response to a previous

missed surveillance.

The second violation occurred because of inadequate work

instructions that failed to prevent cold stressing safety-related piping.

We

were encouraged by the self-identification by maintenance personnel on this

occasion.

The report identifies four examples of weaknesses in the implementation of

your corrective action program.

Specifically, three of these examples are

indicative of a reluctance of station personnel to initiate performance

improvement requests that identify and document conditions adverse to quality.

This issue has been previously identified and documented in NRC reports and

has been the topic of several management meetings conducted during the past

year.

We are concerned that, despite your efforts to improve your corrective

action program, station personnel are still reluctant to utilize the program

to document problems.

The fourth example concerns poor corrective actions

taken to preclude recurrence of an essential service water system waterhammer

event.

In order to fully understand the efforts that you have taken and

future initiatives that you plan to take with your corrective action program,

we request that, following the enforcement conference presently scheduled for

to discuss these issues and others concerning your corrective action program.

.

This meeting was previously discussed with Mr. Maynard.

In addition to the issues mentioned in the previous paragraph, the report

identifies two unresolved items. The first unresolved item (Section 2.2)

,

pertains to problems concerning the adequacy of procuring correct replacement

parts

This issue is of concern because it appears that weaknesses exist in

your procurement process that affects the timely completion of maintenance

conducted on safety-related systems.

The second unresolved item (Section 2.4)

relates to problems identified with timely closure of modification packages

which resulted in an instrument air system failure. This issue is of concern

because weaknesses were identified in your modification program that affect

,

the reliability of safety-related systems. These issues will remain

unresolved pending further NRC review.

Wolf Creek Nuclear Operating Corporation management personnel raised the issue

of blocking the flux doubling feature while in Modes 4 and 5.

Review by

operations management personnel resulted in the determination that the flux

doubling circuitry should not be blocked unless specific actions are taken.

This violation of Technical Specification 3.3.1, Table 3.3-1, Functional

Unit 6.b, was not cited because the criteria identified in paragraph VII.B.2

of 10 CFR Part 2, Appendix C (Enforcement Policy), were satisfied.

Additionally, personnel quickly implemented actions to prevent recurrence.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing.your response to this

Notice, including your proposed corrective actions and the results of future

inspections, NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

It is our understanding that you committed to complete a review of safety-

related motor-operated valves by August 31, 1993, and to identify those valves

which require complete reassembly and a flow test to ensure maintenance was

performed correctly.

If your understanding of this commitment is different

from ours, please contact myself or Mr. Bill Johnson of my staff.

In accordance with 10 CFR Section 2.790 of the NRC's " Rules of Practice," a

copy of this letter and its enclosures will be placed in the NRC Public

Document Room.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
