REQUEST FOR APPROVAL FOR EXTENSION OF TIME FOR HYDR 0 STORAGE, INC.
Attached for your review is correspondence from Pitt-Des Moines, Inc dated September 19, 1991, and December 13, 1991, and a Statement of facts regarding-Quality Assurance Program Approval No. 0668, issued to Hydrostorage, Inc.
The approval holder was billed annual fees under Invoice Nos. AM02268-91 and AM01925-92.
Based on the information furnished, the Office of the Controller recommends that you approve the request for an extension of time to terminate for purposes of avoiding the FY 1991 annual fee to September 19, 1991.
Licensed activities ceased prior to September 30, 1991, and the license was terminated September 4, 1992.
Upon your approval below, Invoice Nos. AM02268-91 and AM01925-92 will be cancelled.

STATEMENT OF FACTS Approval No. 0668 Approval Holder:
Hydrostorage, Inc.
3400 Grand Avenue Pittsburgh, Pennsylvania 15275 Pitt-Des Moines, Inc. notified the Office of the Controller by letters dated September 19, 1991, and December 13, 1991, that Hydrostorage, Inc. (the approval holder) does not exist and had terminated its license with the State Pitt-Des Moines, Inc. requested that Approval of Iowa. On August 20, 1992, No. 0668 be terminated; however, NMSS has confirmed that theletter could h September 19, 1991, f
