Sieber Senior Vice President Nuclear Power Division Duquesne Light Company Post Ofnce Box 4 Shippingport, Pennsylvania 15077 Dear Mr. Sieber: SUBJECT: NRC INSPECTION REPORT NOS. 50-334/93-09 AND 50-412/93-09 AND NOTICE OF VIOLATION This refers to the inspection conducted by Messrs. L. Rossbach, P. Sena, and S. Greenlee of ' this office from April 6 to May 10,1993. The inspection included a review of activities'- authorized for the Beaver Valley Power Station _ in Shippingport, Pennsylvania. ' At the' conclusion of the inspection, the findings were discussed with Mr. D. Spocrry and other members of your staff. _ Areas examined during the inspection are identified in the report. The inspection consisted of document reviews, interviews, and observation of activities important to public health and. _ safety. The purpose of the inspection was to determine whether activities authorized by the license were conducted safely and in accordance with NRC requirements. Based on the results of this inspection, certain of your activities' appeared to be in violation / of NRC requirements, as specified in the enclosed Notice of Violation (Notice). Several high head safety injection pump lube oil piping support brackets were found missing - as a result of our inspection. One of these brackets had been previously identified asfmissing by your staff, but this had not been followed up in a timely manner. Although your staff - responded promptly to our inspector's finding and performed a detailed analysis which showed that the system was operable, we are concerned that the identification of a missing -- bracket by your staff did not receive similar attention. We consider this to be a violation of' NRC corrective action requirements. Also, during this inspection period, your staff identified two examples of failure to adhere to-- radiation protection requirements.'
