Box 499 Columbus, Nebraska 68602-0499 SUBJECT: NRC INSPECTION REPORT 50-298/93-13 ' This refers to the inspection conducted by Mr. R. A. Kopriva, of this office on March 7-11 and March 29 through April 2, 1993. The inspection included a- ' review of activities authorized for your Cooper Nuclear Station facility. At the conclusion of the inspection, the findings were discussed with Mr. R. L. Gardner and those members of your staff identified in the enclosed report. Areas examined during the inspection are identified in the report. Items addressed were the inadvertent isolation of the shutdown cooling (SDC) system, and radiation protection controls for drywell entry. Based on the results of this inspection, two unresolved items were identified. lhe enforcement aspects of these items will be addressed in NRC Inspection Report 50-298/93-17. The potential violations are of concern because the failure ta follow station procedures caused both the loss of power to the shutdown cooling system and a loss of control of workers in a radiation area. Due to the recent plant shut down, the SDC system had been maintaining the reactor in a cold shutdown condition when the SDC system was isolated. This report documents that your short-term reviews to the loss of shutdown cooling did not verify plant response or equipment performance. While short- term reviews were conducted regarding the cause of the event, plant response and equipment performance for this significant event appeared to be deferred to the root cause analysis, which is not performed for up to 30 days. Not verifying plant response or equipment performance until root cause determination is of concern because immediate operational questions will not ! be addressed in a timely manner. l You are not required to respond to this report. You will be asked to address j the actions taken, and planned, for the potential violations in this report as part of your response to NRC Inspection Report 50-298/93-17.
