i Office of Nuclear Reactor Regulation Items of Interest Week Ending May P.1, 1993 l Fermi 2 I Plant management has reviewed the results of the data collected during recent power ascension testing and has determined it is best to limit power until I modifications can be made. When the reactor.is operated at_ above 93-percent power (which is equivalent to a pre-power-uprate-program power-level of 98-percent), there are noticeable increases in turbine component vibration as well as an increase in the pressure pulsation in the 52-inch steam manifold. The licensee is not planning to operate Fermi. 2 at above 93-percent power until after the plant's fourth refueling outage (RF04). The supplier of Fermi 2's main turbine, GEC Alsthom continues model tacting in Rugby, England, to help company engineers design a permanent fix. These hardware modifications are planned for RF04. No outage is scheduled prior to the next refueling in March; however, modifications may'be made before RF04 if the plant shuts down for more than a week for another reason. -During an outage, one possible modification' would be the addition of flow distribution sleeves to the turbine control valves. Current plans call for Fermi 2's power uprate program to be finished next r year. After the modifications are completed in RF04 the plant will operate at 105-percent of the pre-power-uprate-program full-power level. I Yankee Rowe - Two Meetinas Related to Early Component Removal and Public - Concerns to be Held in the Plant Vicinity l The Yankee Atomic Electric Company has proposed removal of certaik plant components (four steam generators, the pressurizer, and reactor vessel internals) before the NRC approves a decommissioning plan for Yankee Rowe. The NRC letter of March 29, 1993, to the Yankee Atomic Electric Company, stated that the staff would meet with the licensee near the plant site to t l discuss the Yankee component removal program. This meeting has now been l scheduled for June 9,1993, a Wednesday, at 1:30 PM in the Monroe Bridge Building (former Simulator Building) in Monroe Bridge, Massachusetts. The March 29 letter also stated that the staff would. hold a public meeting to receive comments from the public. We had stated previously in letters to the Citizens Awareness Network, a local group based near the plant, -that we would l hold such a meeting in response to their request for a hearing. The public l meeting will be held the same day at-7:00 PM and will be located at the i Mohawk Trail Regional High School cafeteria in Buckland, Massachusetts. ~ NRR and Region I staff members will represent the NRC. l j _ Trio to Russia and Finland on Instrumentation and Control ~ Issues' From May 17 through May 27, 1993, Jerry Mauck, Section Chief, Instrumentation and Controls Branch, Division of Reactor Controls and Human Factors is attending several meetings with IAEA in Russia on issues involving reliability and aging of instrumentation and control systems in operating plants. l-I j. 2 i meetings focus on criteria for digital instrumentation and control. system upgrades to existing systems and methods for maintaining digital system reliability. Mr. Mauck will also visit the Finnish' regulatory authorities (STUK) to discuss these issues. Reaualification Proaram Temaorary Instruction Pilot Proaram l During the week of May 17, 1993, the regional operator licensing staffs in l Region V and Region III conducted program evaluations at Diablo Canyon and l Callaway using NRC Inspection Manual Temporary Instruction TI - 2515/117, l " Licensed Operator Requalification Program Evaluation." Brian Hughes of the l Operator Licensing Branch provided headquarters oversight at Diablo Canyon and Dave Desaulniers of the Human Factors Assessment Branch provided oversight at Call away. To date, five of the eight planned pilots for the TI have been compl et ad. Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending May 21, 1993 1 Insoections at Babcock & Wilcox Naval and Commercial The Division of Fuel Cycle Safety and Safeguards staff conducted inspections at Babcock and Wilcox (B&W) Naval Nuclear Fuels Division and B&W Commercial l Nuclear Fuels Plant during the week of May 17, 1993. Inspection areas covered i included item monitoring, accounting and internal controls, alarm resolution, records and reports, physical inventory, and follow-up of unresolved items. Consolidation License Amendment In correspondence dated May 12, 1993, the fuel Cycle Licensing Branch approved- ] an amendment request from Combustion Engineering.(CE) to consolidate all of the licensee's low enriched uranium conversion and fabrication operations at - i their Hematite, Missouri site. Previously, the uranium hexafluoride. [ conversten and pellet fabrication operations were carried out at the Hematite facility, and the pellets were; then shipped to ' Windsor,- Connecticut for fuel-j rod and assembly fabrication.. As a result of this action, fuel fabrication - operations in Windsor are planned to be terminated.in September 1993. However, some fuel research and development activities.will be maintained there. CE management plans to request a separate license for the laboratory operation. The fuel fabrication facility will be decontaminated for release for unrestricted use. j State System of Nuclear Ma' terial Accountancy and Control ~ Assistance to the .j Newly Independent States of the Soviet Union: Coordination Meetina, i May 27-29. 1993 The Chief of the Regulatory and International Safeguards l Branch is\\ I 'partic-l ipating in the International Atomic Energy Agency (IAEA) meeting for. the coordination of member state assistance to the Newly Independent States of. the former Soviet Union ~(NIS) in the area of state. systems of accounting and control of nuclear material.. The Nuclear Regulatory Commission participant' will share information concerning the scope and contemplated activities for the provision of U.S. support to certain NIS countries in developing or enhancing regulatory systems for material control and accounting and physical protection. The meeting will also provide an opportunity to acquire information concerning other country support efforts which would complement the U.S. government support. Audit of Office of Civilian Radioactive Waste Manacement Headauarters L l The Division of High-Level Waste Management quality assurance (QA) staff l l observed a Department of Energy, Office-of Civilian Radioactive Waste Man;gement (OCRWM) internal. audit of their QA program and its implementation at the Forrestal Building in Washington,'DC, during the period May.10-13, 1993. O 2 4 The audit team identified five potential deficiencies, none of which is significant in terms of the overall OCRWM QA program. The Nuclear Regulatory i Commission observers agreed'with the audit team that OCRWM implementation of its QA program is adequate, and that additional management attention should be i given to procedural compliance. l ) Response to RTS Part 21 Report i NMSS is in the process of issuing an Order to nine radiography licensees who l l have received RTS Technology, Inc. (RTS).Model 702. source assemblies within the past six months. The Order modifies ~ their licenses to prohibit use of. these source assemblies. The reason for.this action is that RTS reported to j l the Nuclear Regulatory Commission that several connector sleeves.on Model.702 source assemblies have broken. The cause is attributed by RTS to'the hardness-l specification on the source connector sleeve..A break-in the connector _ sleeve l could lead to an unintentio'nal, and'possibly undetacted, disconnection of the l drive cable from the source assembly. :This disconnection could lead to a licensee invoking emergency procedures.to retrieve an ' unshielded, high i activity radiography source. In the past,. source disconnectionr. have led to i serious overexposures of workers and members of the' public. RM has reduced: i the hardness requirement for future connector sleeves. However, source-assemblies manufactured to the previous hardness specification could have connector sleeves which are too hard. RTS notified NRC that.they sent a j bulletin to each licensee making them aware of the possible defect. The bulletin' listed certain precautions.that must be taken when using the source assemblies. However, because of the consequences of a source disconnection, NMSS has decided to issue an Order that modifies each license to prohibit use i of RTS Model 702 source assemblies with serial numbers: lower than,1867. The-Office of Congressional Affairs, Office of International Programs', Office of l State Programs, Office of Public Affairs, and the Regions have been notified. i i \\ l Aeolied Radiant Eneroy Corooration Sources Tested l I l-In accordance with. license requirements, on April 29, 1993, Applied Radiant 1 l Energy Corporation (ARECO) performed leak tests and " clunk" tests of its 25 l Waste Encapsulation and Storage Facility'(WESF)-sources. All sources passed l the tests. Representatives of the Nuclear Regulatory. Commission (Region II)- and Westinghouse Hanford Corporation (WHC) observed the testing. ARECO, located in Lynchburg, Virginia, leases ~the WESF sources-containing cesium-137,. for use in an underwater irradiator, from the Department of Energy (D0E). WHC-i is the DOE contractor responsible for arranging' for.the. return of WESF sources to DOE from commercial irradiators following the failure of one source at the Radiation Sterilizers, Inc., facility in Decatur, Georgia, in June 1988. Update on'Decommissionino Site - United Nuclear Corporation. Inc.. Montville. Connecticut The United Nuclear _ Corporation, Inc. '(UNC) fuel facility in Montville, Connecticut is being decommissioned. The licensee informed the U.S. Nuclear Regulatory Commission on May 18, 1993, that a sludge sample taken from a-sewage pumping station between the facility and the Montville sewage treatment plant registered-232 pCi/g enriched uranium. Office of Nuclear Regulatory Research Items of Interest Week Ending May 21, 1993 NRC Hiah-level Radioactive Waste Research Proaram Plan On May 19, 1993, Mel Silberberg, Chief of the Waste Management Branch in RES's Division of Regulatory Applications, briefed the Advisory Committee on Nuclear Waste on the current draft of the NRC High-Level Radioactive Waste Research Program Plan. Immediately before Silberberg's briefing, Robert Johnson of NMSS's Division of High-Level Waste Management briefed ACNW on the NRC regulatory strategy for HLW disposal and described how DHLWM expects NRC HLW research to support its HLW regulatory efforts. Silberberg's briefing described the purpose and programmatic and technical bases of NRC's HLW research program, th= curpose of the draft NRC HLW research plan, and the strategy for HLW resec...., including prioritization and responsiveness to DHLWM's user needs. The technical portions of the draft plan were summarized briefly. 10 CFR Part 55. " Operator's Licenses" l The Nuclear Regulatory Commission (NRC) is proposing to amend its regulations to delete the requirements at f 55.57(b)(2)(iv) that each licensed operator pass a comprehensive requalification written examination and an operating test conducted by the NRC during the term of the operator's 6-year license as a prerequisite for license renewal. The proposed amendment at 55.59(c) will require facility licensees to submit copies of each annual operating test or comprehensive written examination used for operator requalification for review by the Commission at least 30 days prior to conducting the examin.ation or the new test. The NRC will retain its authority under S 55.59(a)(2)(jii) to administer comprehensive examinations at its discretion to meet the intent of Section 306 of the Nuclear Waste Policy Act (NWPA) of 1982 and to 'ssure that a licensed operators are being adequately trained and examined in the facility licensee requalification programs. Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending May 21, 1993 Technical Trainina Center (TTC) Staff from NRR/SRXB, the Technical Training Center staff, and an NRR contractor from Oak Ridge National Lab conducted a set of tests at the TTC on May 18 to assist the NRC staff in evaluating the effectiveness and acceptability of the new Emergency Procedure Guidelines (EPGs) proposed by the BWR Owners Group (BWROG). These tests were conducted using the BWR/6 simulator, and involved BWR reactor technology instructors taking operator actions defined by the EPGs and Emergency Operating Procedures (E0Ps). The recent major upgrade of this simulator's software to add an advancad thermal-hydraulic code, a three dimensional core physics model, and high fidelity multi-node containment model made this evaluation possible. The tests included ATWS scenarios beginning at 100% core power and 100% core flow and involving MSIV closures and turbine trips. Other ATWS scenarios were run beginning at the maximum extended operating domain line on the power / flow map. Even though the advanced models on this simulator are incapable of inherently predicting the flow instabilities expected to occur during these events, simulator response was excellent in all other respects. Simulator predictions of core power during severe transient conditiors and suppression pool and containment responses were both coherent and reasonable. These tests were run using both the existing EPGs/EOPs and those proposed by the BWROG. Incident Response Branch (IRB) IRB staff participated in a meeting of the Federal Response Subcommittee. The subcommittee group has been tasked with developing an Annex to th'e Federal Response Plan (FRP) that will incorporate the Federal Radiologicalj Emergency Response Plan (FRERP). This will allow for a single document and philosophy to be used in response to both natural and radiological disasters, thereby singlifying and streamlining response activities. The subcommittee will be working throughout the Summer and early Fall. IRB staff provided training on RTM-92 to members of the Reactor Safety and Protective Measures teams and to Status Officers on May 18. On May 19, IRB provided training on response procedures to Protective Measures team members. During this week, IRB and IRM staff commenced review and evaluation of the proposals received for the Information Management System for the new Operations Center at White Flint-2. Office of Information Resources Management Items of Interests Week Ending May 21, 1993 l Public Comments on Proposed Rule. "10 CFR Parts 40. 72. 74. 75. and 150: Licensee Submittal of Data in Computer Readable Form" The public comments received in response to the rulemaking that would mandate the electronic submittal of nuclear material transaction data focused heavily on the information collection aspects of the proposed rule. Some commenters expressed concern that, contrary to its stated objective, the proposed rule would actually increase the paperwork burden on some licensees. Other commenters believe that transferring data directly from machine to machine would be a more efficient and secure method of submission than mailing files on computer diskettes. There were also comments that suggested additional streamlining of material transaction reporting could be accomplished by adjusting the prescribed reporting schedules. Two cementers provided copies of their letters to NRC's Desk Officer at the Office of Management Budget (OMB). The letters arrived after the OMB 60-day review period mandated by the Paperwork Reduction Act had expired, and after OMB had approved the information collection aspects of the proposed rule. However,- since OMB has the authority to review approved information collections at any time, and is certain to question these collections in view of the public's comments, the Office of Information Resources Management has recommended that the Office of Nuclear Material Safety and Safeguards prepare a response to OMB at the j earliest opportunity. Most of the information needed to respond to OMB should l be available from the resolution of comments that will be prepared for the final rul e. l Scientific Code Distribution 1 \\ The Energy Science and Technology Software Center has established A new program allowing customers to use credit cards (VISA and Mastercard only) 'o order and t pay for codes. This will eliminate the time consuming process of waiting for payment prior to shipping a code order. Codes can now be ordered and shipped within a few days. As in the past, customers can still request expedited shipment modes. Special Pro.iect The File Center completed purging the duplicate license amendment records maintained as part of the Licensing Authority Files. This set of records is no longer needed because all of the involved records are now processed through NUDOCS (Nuclear Documents System). A total of 590 cubic feet of records were reviewed and 290 cubic feet of records were destroyed. Public Comments The Document Control Desk of the Document Management Branch received a submittal l through the Office of Nuclear Material Safety and Safeguards of over 900 letters from the public commenting on a liu.nse application from Louisiana Energy Services for a proposed uranium enrichment plant. Office of Personnel Items of Interest Week Ending May 21, 1993 Recruitment Events Focus on Diversity and Ouality On May 13-14, Janice Clemens and Tom Yingst (RIB) attended the Federal Asian Pacific American Council Congressional Seminar and Training Conference. Speakers included Senator Daniel Akaka, Senator Daniel Inouye, Congressmen Norman Mineta and Robert T. Matsui, and Congresswoman Patsy Mink. The theme of the two-day event was " Harmony in Diversity." The Middle Atlantic Placement Association held their annual conference in Baltimore, Maryland, on May 17-19. Tom Yingst attended workshops that included topics such as AT&T's Total Quality Approach, Adults in Transition, Total Quality College Relations and Recruitment Programs, and New Opportunities for Engineers. Child Development Center Initiatives l On May 18, the NRC Child Development Center, Inc., was notified that its application to be included in the National Capital Area Combined Federal Campaign was approved for the 1993 campaign. Achieving this milestone will enable NRC employees to designate the Center as a recipient of CFC l contributions. i I Office of Enforcement Items of Interest Week Ending May 21, 1993 5 Sionificant Enforcement Actions: A Notice of Violation and Proposed Imposition of Civil Penalty in the amount l of $50,000 was issued on May 17, 1993 to-the GPU Nuclear Corporation. (0yster. Creek Nuclear Generating Station). The action was based on an inadequate temporary procedure change that resulted in a plant heatup above 212 degrees F without certain Technical Specification requirements being met. The violation l has been classified at Severity Level III. After considering the escalation l and mitigation factors of the enforcement policy, a civil penalty equal to that of the base civil penalty for a Severity Level III violation is being proposed. Mitigation for the licensee's identification of the problem and the licensee's good performance was offset by the prior opportunities the licensee had te prevent the violation. The action was based on a Severity' Level III violation involving the licensee's failure to establish and maintain a Quality Management Program (QMP) in accordance with 10 CFR 35.32(a). The $2,500 base civil penalty was escalated based on NRC identification and.the fact.that the licensee had received specific prior notice of the revision to NRC regulations j requiring a QMP. The base civil penalty was _ mitigated based on the licensee's corrective actions. (EN 93-047) A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $50,000 was issued on May 21, 1993 to.the Commonwealth Edison tompany (Braidwood Unit 2). The action was based on the discovery by licdgsee-personnel that contrary to the unit technical specifications, during the previous operating cycle the unit operated without an operable reactor vessel head vent system. Both trains of the system were inoperable due to a closed common manual isolation valve which was not properly aligned to the open position following the previous refueling. This violation _has been classified at Severity Level III. A civil penalty equal to the base civil penalty is i being proposed after offsetting mitigation for the licensee's_ corrective actions and the initiative shown in determining the root cause of this self-disclosing event with escalation for the duration of the inoperable system. (EN 93-048) A Notice of Violation and Proposed Imposition of Civil Penalty in the amount-of $3,750 was issued on May 21, 1993 to University of Michigan, Ann Arbor, Michigan. The action was based on exceeding the licensed power level by'15-percent for approximately 10 minutes. This violation has_been classified at Severity Level III. The base civil penalty of $2,500 has been escalated 50' percent after finding 100 percent escalation appropriate for the licensee's past performance and 50 percent mitigation appropriate for the licensee's corrective actions. (EN 93-049). A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $50,000 was issued on May 21, 1993 to Gulf States Utilities (River Bend MAY 21, 1993 ENCLOSURE K. n w- --r n e nm, g
l h l. Station). The action was based on three violations that occurred while the j licensee was performing activities associated with the containment and drywell airlock interlock mechanisms. These violat4ns have collectively been classified as a Severity Level III problem because they demonstrated the i licensee's lack of understanding of the design basis of equipment that assures the integrity of the containment boundary. A civil penalty equal to that of the base civil penalty is being prop 63ed after offsetting escalation for NRC identification of the violations with mtigation for the licensee's corrective actions. (EN 93-050) An Order Modifying the January 20, 1993, Order Suspending License (Effective Immediately), 58 Fed. Reg. 6825 (February 2, 1993), was issued on May 21, 1993 to Oncology Services Corporation to correct an error in the January 20, 1993 Crder whereby the Lehighton center was identified as not having a copy of the documents incorporated into the license, when in fact it was the Exton center that did not have a copy of the documents incorporated into the license. (EN 93-008A) Civil Penalties Paid: Houston Lighting and Power Company (South Texas) raid the cumulative civil penalty in the amount of $175,000. The first or these three separate actions is based on a number of violations of established procedures which resulted in l the failure to inform NRC licensed operators in the control room of potentially significant conditions that could have affected the operation of the plant. Because the failures to follow established procedures involved plant management personnel, these violations have been classified as a Severity Level III problem. The base civil penalty of $50,000 has been escalated 50 percent to $75,000 after finding 50 percent mitigation appropriate for the licensee's corrective action and 50 percent escalation appropriate for NRC identification of the violations and 50 perceht appropriate for the licensee's prior opportunity to identify one df the violations. The second of the three actions is based on numerous ' examples of personnel errors involving individuals working on equipment on the wrong train or wrong unit. While none of these errors resulted in adverse safety I consequences, collectively they represent a significant regulatory concern and have been classified as a Severity Level III violation. In recognition of the j licensee's corrective actions the civil penalty has been reduced by 50 percent of the base civil penalty to $25,000. The third action is based on the licensee's failure to take corrective actions for a failed motor on a motor operated valve in the Unit 2 Low Head Safety Injection System. The violations I involved in this action have been classified at Severity Level. III because (1) a safety-related valve went unrepaired for 18 months despite multiple opportunities to recognize the significance of the problem, and (2) operations personnel did not recognize the technical specification implications of operating the reactor with the valve inoperable. The civil penalty in this case has been escalated to $75,000 after partially offsetting escalation for the duration of the inoperable valve and the licensee's inadequate corrective actions with mitigation for the licensee's aggressive identification of the root causes of this self-identifying event. (EA 92-175, EA 93-023, EA 93-047) Mercy Catholic Medical Center, Fitzgerald-Mercy and Misericordia Divisions Darby and Philadelphia, Pennsylvania paid the civil penalties in the cumulative amount of $12,500. 0 violation and a Severity Level III problem. -The Severity Level III violation involves a substantial failure to implement the Quality Management' Program, as required by 10 CFR 35.32, in that written directives signed and dated by the authorized user were not prepared prior to administration of_certain radioactive materials to patients. A $6,250. penalty is proposed for this violation. The Severity Level III problem is an aggregate of sixteen-violations.of. the radiation-safety program which collectively-represent a breakdown in the control of licensed activities. A $6,250 penalty is also proposed for this problem. For the first penalty, a basis existed for 50% escalation because the violation.was identified by the NRC, and 100% escalation based on multiple examples. Base-building completion remains at about 95 percent. Sigal Construction Corp. estimates it will be on the site for another two weeks. Automatic sliding doors 'on. front entrance at plaza were. installed. 'Schindler. elevator crew performed punch-list' review and painted elevator door frames. Lincoln County is deemed an affected county under the provisions of the Nuclear Waste Policy Act..The purpose of the meeting was to respond to the representatives' questions concerning the following items: NRC's role in licensing a federal interim storage facility and multiple purpose canisters; NRC's process for determining the acceptability of DOE's NEPA compliance documents, including when and to what extent NRC input will be provided to DOE regarding EIS content, and whether or not multiple EIS's may be used for various facets of the repository system (i.e., repository site EIS and transportation system EIS); the extent to which transportation and socioeconomic issues may or may not be considered by NRC in the repository licensing process; and the extent and nature of conditions that NRC is capable of imposing upon DOE as a consequence of the repository licensing process. CRCPD Annual Meetina The Conference of Radiation Control Program Directors, Inc. (CRCPD) held its annual meeting on May 17-20, 1993, in San Francisco, California. Representatives from the NRC participated in sessions on the NRC/U.S. Environmental Protection Agency (EPA) memorandum of understanding, the NRC compatibility study, the report of investigations of misadministration and incidents, license fees, NRC's site decommissioning management pl.an and the impact on changes to 10 CFR Part 20. NRC also held its public ru'1emaking session with the Agreement States. i Region I Items of Interest Week Ending May 21, 1993 1. Vermont Yankee On May 20,1993, at 6:30 p.m., the Vermont State Nuclear Advisory Panel (VSNAP) held a meeting in Brattleboro, VT. The Director of NRR, Tom Murley, and the Region I Regional Administrator, Tim Martin made presentations and answered questions from the panel. The presentation addressed such issues as LC0 maintenance, license renewal, plant aging and elimination of regulations. NRC's participation at this meeting was at the request of VSNAP and the Governor of Vermont. Representatives from j the New England Coalition on Nuclear Pollution were expected to be in i attendance. Earlier in the day, Dr. Murley and Mr. Martin toured the l Vermont Yankee plant and met with the Governor of Vermont. 2. Wayne General Hospital Enforcement Conference s An enforcement conference was held in Region I on May 19, 1993 with Wayne General Hospital, Wayne, NJ. The conference was held to discuss the apparent violations from an April 13-14, 1993, inspection. The licensee was represented by their president, chief operating officer, radiology manager, radiation safety officer, and physicist. The licensee presented l corrective actions addressing the two major areas of NRC concern: the implementation of the medical Quality Management program and management oversight of the entire radiation safety program. Enforcement action is pending. I 3. Three Mile Island Emeraency Preparedness Exercise \\ A full-participation emergency exercise was held at Thme Mile Island on May 19,1993. The exerrise sinulated a steam generator tube primary to secondary leakage that incmased to the Alert level. Loss of all core cooling subsequently caused a General Emergency declaration. Licensee perfonnance was i good overall. Preliminary, no exercise weaknesses were identified, but some areas for inprovement wem indicated. Chairman Selin Visits Reaion II Sites On May 20, the Chairman and the Regional Administrator met with the f Governor of South Carolina and toured the Westinghouse-facility in. Columbia, South Carolina. The Chairman addressed the Columbia Chapter ~ l of American Nuclear Society in the evening. On May 21, the Chairman and the Regional Administrator toured the. South Carolina Electric and Gas Company's Summer facility and held a press. . conference with local media. 2. Virainia Electric and Power Company On May 20, representatives of the Virginia' Electric and Power Company were in the Region II Office to attend an Open Enforcement Conference regarding disabled auxiliary feedwater. pumps during a reactor trip recovery. 3. Carolina Power and liaht Company On May 21, at'the invitation of Carolina Power and Light Company, nine l congressional legislative assistants from Washington,--D.C. visited the Brunswick site and toured the plant. The legislative assistants represent the Congressmen and Senators from the State of North Carolina. q. i 4. Nuclear Fuel Services. Inc. On May 20, NFS determined, as a result of analyses of sludge samples i from a storage tank at the Waste Water Treatment Facility (WWTF), that the tank had contained approximately 575 grams of uranium 235 as uranyl ) nitrate in solution (175 grams) and uranium oxides'in.the 1/4 inch sludge layer on the bottom (400 grams). This is well below the minimum critical mass quantity, but exceeded the control limit for the tank of-350 grams of uranium 235. The tank,: which is flat-bottomed and of nonfavorable geometry, receives liquids from other nonfavorable and-favorable geometry tanks from the scrap recovery building from the laundry, and from other onsite buildings. The sludge was discovered during an annual inspection of the tank for. sludge. buildup. The licensee is modifying-the WWFT piping to remove the flat-bottomed tank and connect a tank'with a conical bottom that is emptied from the bottom and which contains a spray system to wash down the sides and i prevent solids buildup. This is expected to be completed within ten days.. Because this buildup accumulated over an extended period,-and since there is a material balance of uranium 235 into and out of the ; i tank, the licensee intends to continue to use this tank for the approximately ten day period until'the new tank is piped.in. l .MAY 21, 1993 ENCLOSURE R

Emeroency Preparedness Discussion The Regional Administrator presented a talk on emergency preparedness to an Edison Electric Institute (EEI) sponsored meeting of utility emergency program directors. The meeting was in Green Bay, Wisconsin on May 17, 1993. 2. Palisades Nuclear Plant l The Deputy Director, Division of Reactor Projects and selected members of the Region III and NRR staffs conducted a routine quarterly ) management meeting at the Palisades plant on May 17, 1993, with the Vice l President-Nuclear Generation and other representatives of Consumers Power Company. 3. Byron Nuclear Station The SALP 11 meeting was held on May 18, 1993, at the Byron site. The NRC was represented by the Deputy Regional Administrator; the Deputy i Director, Division of Reactor Projects; the Director, Project Directorate III-I, NRR; and other NRC staff. The licensee j i representatives included the Senior Vice President and Chief Nuclear l Officer; the Site Vice President; and other Commonwealth Edison staff. l Two local television and two local radio stations covered the meeting. 4. Braidwood \\ On May 20, 1993, the Deputy Director, Division of Reactor Projects, Acting Director, Division of Reactor Safety, and other members of the Region III staff met with the Braidwood Site Vice President, Commonwealth Edison Company, to discuss matters of mutual interest. These included long-range management planning, bargaining unit - ) management relationship initiatives, development of an alliance with Westinghouse to address Model D-4 steam generator tube leak problems, and actions relating to a recently identified fuel leak on Braidwood I Unit 2. The iodine concentration in the reactor coolant peaked at 0.55 microcuries/ml and currently is at 0.35 microcuries/ml (the TS limit is l 1.0 microcuries/ml). Because the iodine concentration is trending steadily down, following the historical profile of other fuel leaks, the licensee has decided to continue steady-state operation for the near term in order to assess the extent of the leak and its radiological impacts. Specific criteria, involving evidence of increasing fuel degradation, new fuel leakage, or unexplained radiation level increases, l have been established for removing the unit from service. A decision on ) plant operation for the remainder of the cycle will be based on near term fuel performance. MAY 21, 1993 ENCLOSURE R
h 5. Advanced Medical Systems. Inc. On May 19, 1993, the Chief of the Fuel Cycle and Decommissioning Branch met with Cleveland City Council Members; representatives from the City of Cleveland's Law Department,: Division of Water Pollution Control, Public Services Dept., Public Utilities Dept., and Health Dept.; the Ohio Department of Health; Cuyahoga County Board.of Health; Northeast Ohio Regional Sewer District; and Barry. Koh & Associates (Decommissioning Contractors for'the Sewer District). The meeting was held at the Cleveland City Hall to discuss the radiation survey planned for May 24-28, 1993, to' assess the radiation effects,-if any, from operation of the Advanced Medical Systems, Inc. Facility. The survey is to include direct radiation measurements, soil samples, water samples, I and sanitary sewer sludge samples from the community near AMS. 6. Inaham Medical Center Corporation On May 21, 1993, an Enforcement Conference was conducted in the Region III office between representatives of-Ingham. Medical Center Corporation and members of the. Region III staff. This was an open enforcement conference and it's purpose was to discuss the findings _from the special' safety inspection conducted from February 25 through April 28, 1993. The inspection was conducted to review the circumstances surrounding a misadministration in which 9.9 millicuries of iodine-131 was administered to a patient in error. Five apparent violations were identified during the inspection and discussed at the conference. Cooper Nuclear Station I The Regional Administrator and the Director, Division of Reactor Projects visited the site on May 18, 1993, to conduct a plant tour and to meet with licensee management regarding current outage issues. The licensee is in Day 74 of a refueling outage that began on March 7, 1993, i l and was originally scheduled to last for 56 days. 2. Meetino with the Uranium Producers of America l On May 21, 1993, at 1:00 p.m., the Director, URF0, met in l Corpus Christi, Texas, with the Uranium Producers of America, composed l of representatives of in situ uranium producers licensed by the NRC. Discussions included topics concerning alternatives for excess process fluid disposal from in situ facilities, timeliness of decommissioning, alternate concentration limits, and implementation of the new Part 20 regulations at uranium mills in reclamation. l 3. Comanche Peak Steam Electric Station. Unit 2 - Unclanned Outace On May 20, 1993, an automatic reactor trip of Comanche Peak Steam l Electric Station, Unit 2, from 73 percent power occurred. The reactor trip was initiated due to a less of main generator stator cooling, which caused a turbine and subsequent reactor trip. Plant analysis of the Unit's response to the trip has determined that the main fe.edwater isolation valve to steam generator No. I may not nave closed within time specifications. This is the same valve that did not properl>y respond during a plant transient that resulted in a manual reactor trip on May 4, 1993. Subsequently, to determine the root cause of valve performance, and perform corrective actions, the licensee has decided to begin an outage on June 1, 1993, which was originally scheduled to begin coincident with the completion of initial startup testing on June 11, 1993.
