(1) change the contractor's address and telephone number and (2) add proposed changes to the INDIRECT COST RATES l

in accordance with the DCCA provisional billing rate menos dated February 19, j

1993,-August 20, 1991, and May 29, 1991 as submitted by S. Cohen and Associates. Accordingly, the contract is hereby modified to read as follows:
