NOTICE OF VIOLATION 4

Portland General Electric Company Docket No. 50-344 l

Trojan Nuclear Plant License No. NPF-1 t

During an NRC inspection conducted from March 9 through April 21, 1993, a violation of NRC requirements was identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1993), the violation is listed below:

10 CFR 50, Appendix B, Criterion V states in part, " Activities affecting l

quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances, and shall be l

accomplished in accordance with these instructions, procedures, and drawings."

Trojan Plant Procedure (TPP) 13-5, Revision 0, dated May 29, 1991,

" Control of In-Plant Unsecured Equipment / Material," requires that all j

unattended equipment / material which has sufficient mass and mobility to damage safety-related equipment during a seismic event and is located in i

an area within the scope of the procedure, including the area surrounding the spent fuel pool, be properly secured. Step 4.2.8 states " Portable i

ladders shall be tied down or laid down when left unattended..."

Contrary to the above, on April 27, 1993, three portable ladders had not i

been tied down or laid down and had been left unattended in the area j

surrounding the spent fuel pool.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Portland General Electric Company is hereby required to submit a written statement of explanation to the U. S.

Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C.

20555, with a copy to the Regional Administrator, Region V, and a copy to the

NRC Senior Resident Inspector, Trojan Nuclear Plant, within 30 days of the date of the letter transmitting this Notice of Violation-(Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation, or, if I

contested, the basis for disputing the violation, (2) the corrective steps j

that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the i

license should not be modified, suspended, or revoked or why such other action f

as may be proper should not be taken. Where good cause is shown, consideration may be given to extending the response time.
