Dear Mr. Davis:
This letter is to formally notify you of a scheduled survey by the ASME for the purposes of obtaining an ASME Corporate N Certificate of Authorization, to replace Certificate N-2020, due to expire on August 3,1993. The survey will commence in our Downers Grove offices on June 15,1993 and an exit is currently scheduled for June 16,1993.
By continuing to perform ASME Code activities under a Corporate N Certificate, Commonwealth Edison Company can maintain the public trust by demonstrating to an independent agency our continued ability to safely operate and maintain our nuclear facilities. The Nuclear Survey Team, (only those persons listed below) will assemble for l
a pre-survey meeting to review your Quality Assurance Manual at 2:00 PM on the first date shown above.
The hotel and meeting room is shown on page two.
1 The team will arrive at your facilities the following morning at approximately 9:00 AM to meet with your Staff, including Senior Management, l
to discuss the review of your Manual with possible-changes and conduct the survey.
The team is made up of the following members:

Hartford Steam Boiler Ua have enclosed forms designed to acquaint the survey team with your operations.
Please forward (6) six copies of the completed forms together with a like amount of your Quality Assurance Manual to the attention of the Team Leader no later than 9:00 AM on the date above at the hotel shown.
The original of the completed forms must be given to the Team Leader.
It is understood that you are responsible for arranging for the availability of your Authorized Inspection
- Agency, and/or the Enforcement Authority, including transportation and hotel accommodations if necessary.
During the Survey an applicant may decide to abort the Survey by notifying-the Team (confirmed in writing).
Prior to the decision to abort the survey, the Certificate holder should consult with ASME Staff regarding the options cnd consequences provided in the Subcommittee on Nuclear Accreditation procedures.
j Upon completion or termination of the survey, an ASME invoice will be rendered.
It will include fees & expenses of the ASME team and the Administrative overhead. Certificates and Stamps cannot be issued until' that i
invoice is paid.

By copy of this letter, we are informing the members of the ASME Team of the specifics of travel instructions and accommodations for this survey.
