SUBJECT: NOTICE OF VIOLATION
(NRC INSPECTION REPORT N0. 50-327/93-12 AND 50-328/93-12)

This refers to the inspection conducted by B. R. Crowley of this office on
April 5-9 and 19-23, 1993. The inspection included a review of activities
authorized for your Sequoyah facility. At the conclusion of the inspection,
.
the findings were discussed with those members of your staff identified in the
enclosed report.
i
!
Areas examined during the inspection are identified in the report. Within
-
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
i
activities in progress.
l
Based on the results of this inspection, certain of your activities appeared
!
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violation is of concer, because the inadequate
i
nondestructive examination observed could have resulted in acceptance of a
}
safety-related pipe weld with a significant defect present. The lack of
r
nondestructive test performance in accordance with the required procedure and
code, identified by the NRC based on a limited sample inspection, indicates a
j
weakness in the implementation of your inservice inspection program.
,
In accordance with 10 CFR 2.790 of the NRC's " Rules s f Practice," a copy of
j
this letter and its enclosures will be placed in the ."C Public Document Room.
The responses directed by this letter and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
l
by the Paperwork Reduction Act of 1980, Pub. L. No.96-511.
