14-day Report of a Reportable Event at the University of Virginia Reactor Facility
[ Reactor Docket Number 50-62].
Dear Sir:
Please find in attachment our 14-day report concerning the unintentional and l
inadvertent UVAR Console modification first reported to the NRC on April 29,1993 by telephone, and also,by a special report sent by facsimile. The 14-day report covers the discove'ry of the problem, the initial actions taken, results from a thorough investigation of the root causes Icading to the unplanned change to the reactor console trip electmnics, the current state of the console systems, an assessment of the safety significance of the event, compilation of the license provisions violated and analysis of possible regulatory enforcement ramifications, and the comprehensive corrective actions taken to preclude reoccurrence. The sending of this report fulfills requirements given in UVAR TS 6.4.2.(3) and TS 6.6.1.(2)(b).
This event should not have occurred. Be assured that our concern about it is as great as that of the NRC. Because of this, we have taken the initiative to seek a peer review of our reactor operations program to assure its soundness. Accordingly, we have asked The National Organization of Test, Research, and Training Reactors (TRTR) to send an inspection team to our Facility. We have been advised to expect this group on May 17,1993.

recommendations and to implement eventual additional measures prior to seeking a reactor restart authorization from our Reactor Safety Committee j
(ReSC).
Following this ReSC authorization, and prior to restart of the UVAR, we will discuss the results of our evaluation and implemented l
corrective actions with the NRC Region II Regional Administrator, or his l
designee, in conformance with the NRC Confirmation of Action Letter of l
April 30,1993.
