Gentlemen:
This letter provides Baltimore Gas & Electric Company's (BG&E) assessment of weld inspection activities of three contractor Nondestructive Examination (NDE) inspectors, as discussed in teleconferences on April 23,1993 and April 30, 1993. Attached is a list of safety-related welds inspected by the individuals involved that have been reviewed by BG&E personnel and verified acceptable.
In the teleconferences, we discussed with your staff three instances of problems with NDE visual examinations of welds. All three involved contractor inspectors with personal or health problems that raised questions concerning their fitness for duty. In two of the three cases, the examiners inappropriately accepted welds that were not in conformance with the applicable Weld Authorization Traveller.
We committed to review a sample of each inspector's work to verify the acceptability of these inspections. Of a total of 67 safety-related welds examined by the three inspectors between March 23,1993 and Apri! 17,1993,30 (10 for cach individual) were re-examined. All were found acceptable.
