SUBJECT: NRC INSPECTION REPORT 50-445/93-02; 50-446/93-02 (NOTICE OF
VIOLATION)
This refers to the inspection conducted by Dr. Dale Powers, and the inspectors
identified in the enclosed report, of this office on January 4-30, 1993. The
inspection included a review of activities authorized for your Comanche Peak
Steam Electric Station facility. At the conclusion of the inspection, the
findings were discussed with those members of your staff identified in the
enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, certain activities appeared to be in
violation of NRC requirements, as specified in the enclosed Notice cf
Violation (Notice). The first violation relates to administrative errors made
during Unit 2 preoperational testing. This violation is of concern because it
indicates a failure of supervisors and quality control measures to properly
control work activities. The violation also identifies initial startup and
test procedures that require the performance of referenced activities that are
not clearly defined in the procedures. This part of the violation is of
concern because these unclear references could result in a failure to perform
required activities or the performance of testing with improper plant
conditions.
The second violation involves a repetition of a violation
pertaining to the administrative handling of procedures.
In the example
cited, the control room copy of the procedure was not current.
It is our understanding that, during the exit meeting conducted on January 30,
1993, Mr. Roger D. Walker, Manager, Regulatory Affairs, committed to three
actions in response to the violations. The first action was to have power
ascension training for_ test personnel to assure that they are thoroughly
familiar with the administrative requirements of the program, and to review

test results promptly to provide feedback as to the effectiveness of the
training. The second action was to revise or change initial startup
procedures prior to use to provide a description of each section required to
be performed in all " branch out" procedures referenced. The third action was
to intensify corrective action to ensure control room procedures are current.

me immediately.

You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specified actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
~
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC'.s " Rules of Practice," a copy of
this letter and its enclosures will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget, as
required by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
