SUBJECT: NRC INSPECTION REPORT 50-482/93-01 (NOTICE OF VIOLATION)

This letter refers to the inspection conducted by Messrs. G. A. Pick and

L. E. Myers of this office for the period of January 3 through Fet'ruary 13,

1993.

The inspection included a review of activities authorized for your Wolf

Creek Generating Station. At the conclusion of the inspection, the findings

were discussed with those members of your staff identified in the enclosed

report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examination of procedures

and representative records, interviews with personnel, and observations of

activities in progress.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

Violation A includes two examples of failure to follow

pro mdures. We are concerned with this violation because it indicates

contu mg problems with inattention to detail. Violation B pertains to an

inadequate test procedure resulting in an inadvertent reduction of the

refueling water storage tank level. The results of the event were not

significant, and your personnel implemented thorough corrective actions.

However, we are concerned because several combined deficiencies caused the

event.

Wolf Creek Nuclear Operating Corporation personnel determined that several

local alarm response procedures were missing due to errors in procedure

distribution.

This could have resulted in difficulty responding to equipment

problems. This violation of Technical Specification 6.8.1.a was not cited

because the criteria identified in paragraph VII.B.2 of 10 CFR 2. Appendix C

(Enforcement Policy), were satisfied. Wolf Creek Nuclear Operating

Corporation personnel promptly implemented effective actions to prevent

recurrence. The inspectors identified an instance of Wolf Creek Nuclear

Operating Corporation personnel signing onto an incorrect radiation work

permit.

This violation of Technical Specification 6.8.1.a was not cited

because the criteria identified in paragraph VII.B.2 of 10 CFR 2, Appendix C

recurrence.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

It is our understanding that you committed to troubleshoot the emergency

diesel generator starting air system during Refuel VI to identify the root

cause of the failure of the emergency diesel generator to stop.

If your

understanding of this commitment is different than ours, please contact the

Section Chief or myself.

In accordance with 10 CFR 2.790 of the NRC's * Rules of Practice," a copy of

this letter and its enclosures will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, Pub. L. No.96-511.

to discuss them with you.
