b NOTICE OF VIOLATION South Carolina Electric & Gas Docket No. 50-395 V. C Summer Nuclear Station License No. NPF-12 During the Nuclear Regulatory Commission (NRC) inspection conducted on May 1 -

31, 1990, a violation of NRC requirements was identified.

In accordance with h

the " General Statement of Policy and Procedure for NRC Enforcement Actions,"

,u 10 CFR Part 2, Appendix C (1990), the violation is listed below:

Technical Specification 6.8.1.a requires that written procedures be y

established, implemented, and maintained for activities referenced in Appendix A of Regulatory Guide 1.33, Rev. 2, February 1978.

Electrical Ll Maintenance Procedure. Agastat Relay Replacement, Rev. 4 EMP-300.012,-

o requires that replacement and reconnection of the relay wires be recorded 4

and verified by two electrical technicians and a quality control inspector.

Contrary to the above, on April 27, 1990, an improperly installed Agastat relay to the 10A bus under voltage trip control logic resulted in an ESF actuation of the A diesel generator when an under voltage signal was present when it actually did not exist.

The root cause of the improperly-installed relay was inadequate verification by the electrical technicians and the Quality Control' inspector of the work activities as described in the Electrical Maintenance Procedure.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, South Carolina Electric & Gas Co.

is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, V.C. Summer Nuclear Station, within 30 days of the date of m

the letter transmitting this Notice. This reply should be clearly marked as a

" Reply to a Notice of Violation" and should include: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective v

steps which have been taken and the results. achieved, (4) the corrective steps J

which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will'be given to extending the response time.

If,an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause 9006200261 900611 ADOCK 0300gb.
