Notification of Change in Senior Operator Status, Haddam Neck Gentlement The following licensed operator of Connecticut Yankee'has developed a medical condition that has resulted in his failure to meet the medical requirements of 10 CFR 55.21.
Emil L. Montalvo License # SOP-4384-3 Northeast Nuclear Energy Company became aware of this medical condition on May 11, 1990.
Pursuant to 10 CFR 55.33(a) (1), a conditional license is requested.
A Certification of Medical Examination by Facility Licensee, NRC Form 396, in support of this request is being submitted to the Regional-Administrator, NRC Region 1.
