Dear Mr. Lee:

Enclosed is-the NPDES permit renewal application for the Perry Nuclear Power Plant.

Included in this application are the following documents:

(1) Application Form 1 - General Information (2) Application Form 2C - Wastewater Discharge Information (3) USCS Map (Perry Quadrangle, Lake County) showing the location of the plant and associated outfalls.

L (4) Line Drawing of Water-Flow at the plant site.

(5) Check for the $15,00 Permit Renewal Application Fee.

Please note that no data (Tables V-A,-V-B, and V-C) has been included for Outfall 002, Chemical Cleaning Lagoon, because there was no discharge from this outfall in the past 12 months. However, there is potential for future discharges so this outfall should continue to be permitted.
