INSERVICE TESTING PROGRAM CHECK VALVE - EXERCISE OPEN/ FULL FLOW TESTING DOCKET NO. 50-354.
HOPE CREEK GENERATING STATION Public Service Electric and Gas Company (PSE&G) has determined that, contrary to a commitment made in our letter, NLR-N88205, I
dated December 23, 1988, which also transmitted Revision 2 to our 4
P IST Program, testing procedures to demonstrate full (safety function) flow for certain check valves have not been revised.
As described in the above letter, based upon NRC concerns which surfaced in a June-29-30, 1988-meeting, PSE&G made a commitment
'to revise appropriate test procedures'to demonstrate that tested check valves were. capable of passing the flow required for.their safety functions.
Test procedures addressing most affected check-1 valves ~were revised.
The remaining valves were found to require system modifications or flow calculations / extrapolations,'as well as procedure changes, to perform the proper flow testing.
Through.an. administrative error, the tracking of'the commitment L
for all check valves was closed while the procedures for those valves requiring system modifications and/or flow calculations had not been revised and continued to perform only exercise-open s
testing.
This is a failure to completely implement our-commitment.

This-discovery led to a complete review of the issue l
with the following results...of the 93 check valves initially.
identified as requiring full flow testing,.53 are. currently being properly tested and 12-have no 'open' safety function and were requested-to be deleted in our IST Program, Revision 2 submittal.
The remaining 28 check valves were found to require either system modifications or innovative testing techniques to be developed,
.as well'as procedural revisions, to accomodate full flow testing.
PSE&G'will-complete all those changes necessary to satisfy our commitment by June 30, 1990.

'4LR-N9010 5 This problem is considered to be an isolated case of erroneously closing a task in our computer-aided commitment tracking system.
However, to prevent recurrence, we are conducting a root cause analysis and evaluating the incident for any programmatic flaws.
