_j DPR-69 During an NRC inspection conducted March 25, 1990 through April 28, 1990,.and in -accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C), the following violation was identified.

Technical-Specifications 6.8.1 states that written procedures shall be l

established, maintained and implemented covering the applicable procedures i

recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, February

'1978.

' Appendix A of Regulatory Guide 1.33, Rev. 2,-1978, requires administrative procedures for equipment control (e.g. locking and tagging).. Administra-tive procedure CCI-309B requires that the operation of any locked valve be 4

annotated and approved in the " Locked Valve Deviation Log".

Contrary-to the above, on' April 20, 1990, three valves were unlocked and repositioned in the waste gas decay system without adequate documentation and verification which resulted in an unplanned release of the #13 waste-gas decay tank.

l This is a Severity. Level IV violation (Supplement I).

Pursuant to the' provisions of 10 CFR 2.201, Baltimore Gas and Electric Company is hereby required to submit to this office within thirty days of. the letter 1

which transmitted this Notice, a written statement or explanation in reply, R

including:

(.1) the corrective steps which have been taken and the results achieved; (2)-corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Where good cause is
