Quality Assurance Program Approval for Radioactive Material Packages

Dear Mr. Mcdonald:
According to direction provided by 10CFR71, Subpart H, we hereby respectfully request revision of the referenced NRC approval of the description of our Quality Assurance Program. At the same time, since our existing program approval does noi expire until December of this year,we would also like to request an early renewal of our OA Program.
Included with this document, please find revision 5 of our letter number OA-781 which contains a synopsis of our Quality Assurance Program. The main reason for the revision of our QA program description was to describe our expanded organizational structure. We have also included a check in the amount of $150.00 to cover the cost of the requested action.
Please advise if any additional inforntation is desired or if you have any questions concerning this submittal.
