4 NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-265 Quad Cities Unit 2 As a result of the inspection conducted from March 18 through April 28, 1990, and in accordance with the " General Statement of Policy) and Procedure for NRC Enforcement Actions," 10 CFR Part 2 Appendix C, (1990 (Enforcement policy) the following violation was identified:

10 CFR 50 Appendix B, Criterion V, in part states that " activities affecting quality shall be prescribed by documented instructions, procedures, or drawings", and "shall include appropriate quantitative or qualitative acceptance criteria for determining that activities important to safety have been accomplished".

Contrary to the above, on February 20, 1990, a Residual Heat Removal (RHR)

System drain line was inadvertently cut and capped after a contractor. foreman f ailed to properly identify the full run of a jet pump instrument drain line -

during a pre-work walkdown. Subsequently, a contractor Quality Control inspector initialed the work package certifying that-the jet pump line identification was complete and correct, but he too failed to walkdown the L

entire run of piping. The work package did not contain instructions describing an acceptable technique for identifying the pipe, nor did it include acceptance criteria for the Q.C. inspector.

(1)correctiveaction taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Ccasideration may be given to extending your response time for good cause shown.
