I At a pproximately 3:30 pm (local time) on April 24, 1990, TU Electric syncironized Comanche Peak, Unit 1 to the grid., After producing some 274 NW hours of electricity (nct) with a peak of approximately 100 MWe, the unit was taken off the grid in order to correct two leaking check valves in the

auxiliary feedwater system. The plant remained at approximately 8% power..

While the check valves were resented by initiating full forward flow and then J

abruptly terminating it to provide positive seating pressure. Power ascension s

is continuing and the licensee expects to reach the 50% test plateau in.

mid-May.

At approximately 4: 55pm(localtime)onApril 19, 1990, Comanche Peak,-

Unit 1 entered Mode 1 for the first time. The licensee, TU Electric, estimates that 12 weeks will be required for power ascension to accomodate 1

testing and evaluations, including a critical self-assessment at.the 50% test plateau. Thus, Unit 1 is expected to be placed into commercial operation-in mid-July 1990.

Meeting With DOE on Decomissioning Research On April 19.-1990, staff from RES and NMSS met with DOE staff and consultants to discuss potential joint DOE /NRC sponsored research associated with assessing the effectiveness of efforts to decontaminate soils and structures before decomissioning. Agreementwasreachedtoworktogether.to(1) develop-technical guidance for conducting radiological site surveys to determine.

compliance with decommissioning forNRC(GENII)andDOE(RESRAD) guidelines,(2)comparecomputercodesdeveloped which are used for translating residual-radioactivitylevelstoannualradiationdose,and(3)comparein)utparameters and output from the GENII code with actual field measurements. Tle GENII code is now meing used by PNL, under contract to RES, to develop residual contamination limits which would be acceptable to NRC for decomissioning nuclear facilities.

Initial work is expected to begin in thelnext few weeks. Code comparisons and development of guidance for conducting radiological site surveys should be completed in the next 6 months. Comparison of. output from the GEN 11 code with actual field measurements is. anticipated to take about 2 years to complete.

M_ichigan Challenges LLW Act The State of Michigan filed a lawsuit of April 18, 1990. challenging the constitutionality of the Low-Level Radioactive Waste Policy Amendments Act of 1983. The lawsuit raises many of the same points in the suits filed by the State of New York and the citizens' group in Nebraska.

Governor Blanchard had announced the State's intention to file a lawsuit on February 9,1990.

Request to Import Namibian Uranium Edlow International Company has submitted an application for the import of 500,000 kilograms of Namibian-origin uranium. This is the first request for a license to import material from Namibia since they became independent from South Africa on March 21, 1990.

U.S.~ Government regulations are being amended to lift U.S. economic sanctions imposed on Namibia pursuant to the Comprehensive Anti-Apartheid Act of 1986.

The application is being forwarded to the Department of State for review; upon receipt of Executive Branch views, the application will be transmitted to the Comission for consideration.

Items of_ Interest Week Ending April 27, 1990 Browns Ferry' Nuclear Plant, Unit 2 A number of TVA employee concerns at Watts Bar and at Sequoyah indicated that.

cable-pulling practices. employed at these sites violated plant procedures; and l

furthermore the procedures themselves were not adequate to ensure cable inte-grity during installation. Actual installation damage to electrical cables l

was discovered at Watts Bar. These cables appear.to have been damaged by; excessive tension applied during installation (i.e. pullby damage). Like Sequoyah, TVA implemented a wet conduit high potential testing program to discover.if-cable pullby damage existed at Browns Ferry.

TVA selected the worst case cables of ten conduits at Browns Ferry to ground

tested while wetted. Results from testing:the third conduit, on April 13th and 14th of this year, revealed significant insulation damage to one. cable.

(i.e. the jacket and insulation were cut all the way through, baring two 'of the.

conductors). Subsequent testing of the foe. ch conduit, on April'18th, exhibited results of anomalous behavior:in one of the 79 total conductors. Although TVA is still investigating, their preliminary analyses seem to indicate-that neither finding is an example of pullby damage. The NRC staff is closely l

monitoring TVA's activities and plans to evaluate the-licensee's-analyses, including a physical examination of the damaged cable (s) as part of a cable l

installation team inspection previously scheduled to be onsite April 23, 1990.

The staff has requested TVA personnel at Browns Ferry to temporarily forego cable repairs or preserve the-damaged portion until the arrival of the NRC team next week.

Calvert Cliffs Units 1 and 2 l

On April 13, 1990, Unit I achieved criticality, increased the power level, and was paralleled to the grid. The unit attained 100% power on April-21, 1990, and began shutdown the following day for a scheduled maintenance outage.

The unit experienced two. Unusual Events during the short operating period. The first was the result of three of the four reactor protection. system over power delta-T channels being declared in-operable =and the second when three of four of the delta-T calculators were found out of set point. band. Procedure and setpoint changes were reviewed and approved by the site safety committee and the unit power ascension resumed subsequent to the corrective actions.- The i

unit commenced shutdown on April 22, 1990, for a scheduled maintenance /

surveillance outage and is currently scheduled to restart on July 19, 1990.

L Plant Status as of 04/25/90:

i Unit 1:

The Unit is shutdown and has comenced the scheduled outage. The major items to be accomplished during the outage include eddy I

current testing of the steam generators, reactor coolant pump seal replacements, saltwater header modification and low pressure safety injection seal modifications.

Current scheduled restart is July 19, 1990.

2-x Unit 2:

Cold Shutdown, Defueled - Extended refueling outage since 03/18/90.

The licensee completed evaluating the cause of cracks in the pressurizer heater sleeves and stated that they resulted from primary water stress corrosion which was initiated by. excessive:

reaming of the' sleeves following. pressurizer fabrication. B&W and BG&E have developed a repair plan which has been reviewed by NRR-and Region I.

Repairs to the pressurizer are expected to be completed with plant restarting near the end of the; year.

Comanche Peak Steam Electric Station : Unit 1 At approximately 4:55 pm (local time) on April.19,1990, Comanche Peak, 4

l Three Mile' Island Unit 1 L

The staff met with GPU Nuclear on April 20 to' discuss the causes of and options to mitigate fouling of B&W Once Through Steam Generators (OTSGs) by secondary system corrosion products. Power level was-limited to about 80% at'TMI-1 following the January-February refueling outage due to this 1henomenon.

GPU Nuclear, after evaluating the options available.to correct: tie problem, elected to perform a special test under 10 CFR 50.59 on March 30 that involved.a manual' reactor trip, stopping and restarting reactor coolant pumps and manually initiating emergency feedwater flow. During the meeting, GPU Nuclear; emphasized that the fouling problem is very unpredictable and;that, based on past history, was not expected to be a problem during this fuel. cycle. They i

L also discussed the advantages and disadvantages of the options to alleviate the problem, both short-term and long-term. GPU Nuclear is planning to perform chemical cleaning of the OTSGs during the next refueling outage pending the' results of a plant-specific materials testing program. They stated that this is the only presently known method of removing significant quantities of-corrosion products already present in the OTSGs and has been successfully employed at Oconee Units 1 and 2.

Limited success has been realized in the past using a " water slap" process developed by the B&W owners. Although the reactor trip option has been quite successful the three times it has been l.

. employed, it is not GPU Nuclear's preferred option as a long-term solution to the problem. GPU Nuclear did not, however, eliminate it as one of the i

options that may need to be employed in the future as a short-term measure.

T1e staff. encouraged chemical cleaning..with one potential technical problem to be considered, as'the preferred corrective action but did not. raise any significant technical concerns regarding the reactor. trip option.

A-representative of-the B&W Owners Group stated that the reactor trip option'is not contrary to their Safety PerformanceL and Improvement Program.

1 vs Office of Nuclear Mater.ial Safety and Safeguards Items.of Interest Week Ending April 27, 1990 INDUSTRIAL AND MEDICAL NUCLEAR SAFETY Fuel Cycle Safety West Valley Demonstration Project The Department of Energy-(00E) is negotiating a Federal-Facilities Compliance Agreement with the Environmental Protection Agency (EPA) for the West Valley Demonstration Project. This agreement would be analogous to some.already-

~

~ reached between the two agencies for other DOE facilities.

It would specify measures to be taken by DOE to bring West Valley into compliance with EPA regulations on hazardous and mixed wastes, and would assign responsibility to New. York State for various aspects of environmental monitoring'and-enforcement.

DOE has' prepared a draft agreement to present to EPA'in a formal negotiating session on May 9, 1990.- DOE has invited NRC to send a representative to the May 9. meeting.

It is-possible that some aspects of the DOE / EPA agreement.will-have an impact on the condition of the West Valley site at the termination of l.

the Project, when the site will presumably revert to NRC licensing authority.

LOW-LEVEL WASTE MANAGEMENT 1

Visit to Texas Instruments Inc.

On April 24 representatives from the Division of Low-Level Waste Manegement-and Decomissioning and Region I visited the Texas -Instruments, Inc. facility located in North Attleboro, MA to discuss the status of.decomissioning and-criteria with the licensee and to measure radiation levels of the site. Texas Instruments, Inc. fabricated uranium fuel for the U.S. Navy and for; comercial customers until 1983 and concluded site decontamination efforts the same year.

Additional decontamination in two small areas is recomended by the. staff on the basis of a survey conducted by NRC contractor Oak Ridge Associated Universities. The schedule for completing decomissioning will be ~ addressed by the licensee in a letter expected in May 1990.

HIGH-LEVEL WASTE MANAGEMENT Seismic Hazards Meeting A technical exchange among the Department of Energy (DOE), NRC, and the Nuclear

~ Waste Technical Review Board (NWTRB) was held in Las Vegas, Nevada on April 12, 1990. The purpose of the exchange was to give both DOE and NRC the opportunity to brief NWTRB on the status of their approaches'to investigations and analyses of seismic hazards at the proposed high-level waste geologic repository at Yucca Mountain, Nevada.

DOE explained that it considers the seismic hazard procedures in Appendix A' of 10 CFR Part 100 inappropriate for a geologic repository and that it is developing a seismic hazard approach that' incorporates both deterministic and probabilistic methodologies. NRC stated that it is developing.a-proposed ~

. technical position that would consider a deterministic seismic hazard evalua--

tion, supplemented by e probabilistic seismic hazard. analysis, an acceptable approach to fulfilling NRC and EPA ~ requirements.

NRC also explained that its technical position on seismic hazard investigation methods is being issued -not out of dissatisfaction with DOE's planned seismic hazard investigations,'but rather as input to NRC's License Application Review Plan,

Public Meeting on MOV Test 'Results.

sponsored a public meeting on' April 18, 1990 to review and discuss the results of motor-operated gate. valve (MOV) tests with a panel of outside experts..The>

1 panel consisted of 22 members with representatives from the' utilities --and=.

valve, motor operator:and diagnostic equipment manufacturers. -Representatives j

from NSSS vendors, an A&E firm,' national standards connittees,- a utility user 9

- group,theElectric-PowerResearchInstitute>(EPRI)andtheIdahoNational-Engineering Laboratory (INEL) were also on the )anel. Seven NRC management and staff were also on the panel and took part in t1e review and discussions.

There was a total of 155-attendees at the meeting (22. expert panelists;L7 NRC-panel members and 126 in the general audience).

The testing program was conducted to determine whether specific types of motor operated gate valves in BWR plants lwill close against high velocity flows that can develop in the event of-a full guillotine pipe: break,outside containment.

'i The results are contributing to the. technical basis for. addressing Generic-Safety Issue-(GSI) 87, " Failure of HPCI Steam Line Without Isolation," and for-implementing Generic Letter (GL) 89-10, " Safety-Related Motor-Operated Valve Testing and Surveillance."

It is important to realize that although the valves closed in almost all of' the blowdown tests, the motor operators on the valves-had been purposely set l

to deliver much larger thrusts than would normally be delivered.using current.

industry practices. This procedure was established so.that the force components that have to be overcome by the motor operator during closure could.be measured i

and quantified.

The test results reconfirm earlier research results-and. provide additional data that the thrust equation typically used to size motor operators for these types of valves under differential pressure loads should not be applied to those valves which may experience severe damage during closure against large differential-4 pressure loads. The results also show that the same thrust equation is more appropriate for estimating the closure thrust for gate valves not~ expected to.

l experience significant internal damage during closure.

However, for those-latter l

valves, the friction factor of 0.3 normally used for estimating the disc friction-force in the thrust equation may not be-conservative under high velocity flow loads. A friction factor of approximately 0.5 appears to provide closer agreement i

i between predicted and measured disc friction forces for those latter valves.

f In general, the panel agreed with the results of the tests; however, some l

commented that the data should be made available for industry use.

INEL had been previously instructed by RES to make the data available to the NRC Public Document Room and also to take steps for making copies of the magnetic test -

L data available for industry use.through the DOE Office of Technology Transfer at INEL. This satisfied the panel members requests.

l Nuclear Waste Technical Review Board Meetings-Jacob Philip and Edward O'Donnell of the Waste Management Branch, Division of Engineering, RES attended two meetings of the Nuclear Waste Technical Review t

Board (TRB). The TRB is a recently formed board, appointed by the President, to review the technical aspects of DOE's program for disposal of civilian high level radioactive waste. On April 7th the TRB heard a status report from DOE s

on its study evaluating different alternatives for designing and developing an exploratory shaft facility (ESF). The ESF will be an underground facility at Yucca Mountain, Nevada, where DOE will conduct experiments to determine the suitability of Yucca Mountain for use as a geologic repository for civilian high. level radioactive waste disposal. DOE.is studying 17 different options for the ESF and repository configuration. On April 12th the TRB conducted a technical information exchange with representatives of DOE, NRC, and the State of Nevada to discuss seismic hazerd issues that might impact a geologic repository at Yucca Mountain. ' Technical presentations on seismic' hazards were made by. DOE and Nevada. While DOE's definition of the seismic hazards at Yucca Mountain is-based on movements of specific faults in proximity to Yucca Mountain, the State of Nevada stressed the need for considering the entire Basin and Range Province (ofwhichYuccaMountainisapart)toassess-theseismichazard. Philip S. Justus and Abou-Bakr Ibraham, NMSS, spoke on NRC's approach on regulatory guidance for seismic hazards assessment.

Division of Regulatory Applications Meeting With DOE on Decomissioning Research l

On April 19, 1990, staff from RES and NMSS met with DOE staff and consultants to discuss potential joint DOE /NRC sponsored-research associated with assessing the effectiveness of efforts to decontaminate soils and structures before decommissioning. Agreement was reached to work together to (1) develop technical guidance for conducting radiological site surveys to determine for NRC (GENII) and DOE (RESRAD) guidelines, (2) compare computer codes compliance with decommissioning which are used for translating residual radioactivity levels to annual radiation dose, and (3) compare input parameters and output from the GENII code with actual field measurements. The GENII code is now being used by PNL, under contract to RES, to develop residual contamination limits which would be acceptable to NRC for decommissioning nuclear facilities.

Initial work is expected to begin in the next few weeks. Code comparisons and development of guidance for conducting radiological site surveys should be completed in the next 6 months. Comparison of-output from the GENII code with actual field measurements is anticipated to take about 2 years to

Division of Operational Assessment (DOA).

On April.26, 1990 an Advance Copy of NUREG-1405, " Inadvertent Shipment _of a.

Radiographic Scurce from Korea to Amersham Corporation, Burlington,-Massachusetts"

.was distributed to the Comissioners._ The report will be released to the-public on May 7,_1990.

The Vogtle IIT team is continuing report preparation. The report is expected-to be available by May 31-to support a June 8 Comission briefing.

Al Gibson from Region II began a four month detail. assignment with AE0D this-week to manage the Zion Diagnostic Evaluation Team.

AE0D attended the initial meeting of the Standing Comittee on Energy of Lthe

Policy Coordinating Comittee on Emergency Preparedness / Mobilization Planning.

Other representatives were from DOE, D0D, 00J, 001, DOT,(Treasury, CIA, FBI, FEMA, NCS, NSC. The mission of the Subcomittee is to:

1)examinethe vulnerability of U.S. energy systems and to. propose options to reduce this' vulnerability;-(2) consider options to mitigate the impacts.of disruptions of the U.S. energy systems, should they occur; and (3) develop and maintain an information base and forecasting methodology for assessing the potential impacts of energy supply disruptions, and for preparing. analyses of the impacts of such disruptions, should they occur.

On April 27, the Emergency Response Data 1 System was delivered to the Operations Center. Installation and testing will be delayed because of concurrent use of the facility by the Vogtle IIT team.

Division of Safety Programs The fourth quarter CY1989 A0 Report (NUREG-0090, Vol. 12, No'. 4) was delivered to Congress on April 17, 1990. The report contained two A0s at NRC licensees; one diagnostic and one therapeutic; one other event of interest (Significant fuel clad degradation at Haddam Neck; and, one update to a previously reported A0.

'c Office of Governmental and Public Affairs Items of Interest Week Ending April 27, 1990 State Programs Kentucky Radiation Control Program Reviw R. L. Woodruff, Region 11 State Agreements Officer, conducted a review of the Kentucky Radiation Control Program on April 23-27, 1990 in Frankfort, Kentucky. An exit meeting was held with Harry J. Cowherd. M.D.,

Secretary, Cabinet for Human Resources, on April 27. Staff findings will be forthcoming.

Northwest Interstate LLW Management Compact Comittee Meeting Dean Kunihiro, Region Y State Liaison Officer, attended the Northwest 1

Interstate Compact on Low-Level Radioactive Waste Management Comittee i

meeting on April 23, 1990 in' Honolulu, Hawaii. The Comittee discussed a 4

possible disposal agreement with a representative from the. Rocky Mountain LLW Compact although no timetable was established. The representatives at the meeting cited this possible disposal agreement as an example of States taking the consolidation of sites in their own hands and why the Federal government should not intercede. The Committee was briefed on the status of below regulatory concern, mixed waste and Pathfinder reactor decomissioning, and waste disposition.

LLW Forum Meeting The low-Level Waste Forum held a quarterly meeting on April 25-27, 1990 in Austin, Texas. The Forum reviewed pending litigation which.could affect the LLW siting process including the lawsuits filed by New York, Michigan, citizens in Nebraska and an Alabama hazardous waste ruling.

The Forum also discussed mixed waste, below regulatory concern matters, LLW compliance and an update on 0 activities including the Agreement State guidelines, electronic dattn ie rulemaking standard review and content guidance, and im)11 cations of the BEIR V Report. The Forum also held a special session w11ch examined the implications of the number'of sites being developed addressing such issues as health and safety, economic viability, transportation safety, political acceptability and federal vs. state preeminence.

Meeting with DOE Officials from Savannah River Stewart Ebneter, Region II Administrator, and Robert Trojanowski, Region 11 State Liaison Officer, met with Department of Energy officials at Savannah River on April 25, 1990. The purpose of the meeting was to discuss mutual support for emergency planning and other issues of interest.

I Michigan Challenges LLW Act i

The State of Michigan filed a lawsuit of April 18, 1990 challenging the constitutionality of the Low-Level Radioactive Waste Policy Amendments Act of 1985. The lawsuit raises many of the same points in the suits filed by the State of New York and the citizens' group in Nebraska.

Governor Blanchard had announced the State's intention to file a lawsuit on February 9,1990.

International Programs Senior IAEA Visitor On April 25 Mr. Noramly bin Muslim, IAEA Deputy Director for Technical Cooperation (TC), met with Comissioner Remick, GPA Director Harold Denton, NMSS Division of Industrial and Medical Nuclear Safety Richard Cunningham, and other GPA personnel to discuss ongoing and projected IAEA activities in the technical cooperation area. Mr. Noramly shared his view of Agency TC priorities and was briefed on NRC's international priorities. He noted that his group is dealing with an " avalanche" of requests for safety support to eastern European countries following the dramatic changes there. Mr. Noramly also expressed appreciation for NRC's support of the TC program over the years and hopes that it will continue.

AIT-CCNAA Joint Committee Meeting On April 26-27, representatives from NRR, RES and GPA/IP attended the annual American Institute on Taiwan (AIT) - Coordination Council for North American Affairs (CCNAA) Joint Committee Meeting hosted this year in Bethesda by AIT. The Taiwan team provided a series of briefings on their reactor power program, disposal of nuclear waste, and isotope production ca) ability. Thomas Murley addressed Commission priorities and Themis Speis lighlighted research programs in support of key regulatory

issues, in reviewing cooperation projects, NRC proposed a new topic for cooperation in the area of " Operational Safety" that would provide a more balanced exchange of information and would place emphasis on NRC learning more from the operational experience of Taiwan's six U.S.-designed PWR and BWRs.

Request to Import Namibian Uranium Edlow International Company has submitted an application for the import of 500,000 kilograms of Namibian-origin uranium. This is the first request for a license to import material from Namibia since they became independent from South Africa on March 21, 1990.

U.S. Government regulations are being amended to lift U.S. economic sanctions imposed on Namibia pursuant to the Comprehensive Anti-Apartheid Act of 1986. The application is being forwarded to the Department of State for review; upon receipt of Executive Branch views, the application will be transmitted to the Commission for consideration.

An Order Imposing Civil Penalty in the amount of $8,750 was issued on to the Veterans Administration Medical Center Brooklyn',

' April 20, 1990 New York. Theactionwasbasedonviolationsinvolvingthefailureto follow emergency procedures for a teletherapy unit and a lack of management oversight of the facility's licensed programs.

The licensee responded to the Notice in a letter dated January 3,1989.

After careful consideration of the licensee's response, the staff concluded that the violations occurred as stated with the exception of one viola-tion,whichwasamended,andthatthelicenseeprovidedinsufficient justification for mitigation of the civil penalties.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $6,250 was issued on April 20, 1990, to MQS, Inspection, Inc.,

Elk Grove Village, Illinois. The action was based on an individual receiving a dose of 4.490 rems during the fourth quarter of 1989, a dose in excess of regulatory limit of 3 rems, and failures to: make an adequate survey of a radiography device to ensure the source'had returned to its shielded position, lock the radiography device after assuring that source was in a safe position, and make a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> report of an event that threatened to cause an exposure in excess of 5 rems to the whole body.

These violations are of concern as they indicate a lack of adherence to NRC requirements which focus on-safely performing radiography operations and prevention of overexposures. The escalation and mitigation factors in the Enforcement Policy were considered and a 25% escalation of the base civil penalty was deemed appropriate. The 25% mitigation warranted-for licensee's prompt and adequate, but not extensive corrective action was offset by the 50% escalation warranted for the licensee's past performance l

which includes a civil penalty for a similar event in the last 18 months.

No further adjustment of the civil penalty was deemed appropriate.

Civil Penalty Paid i

Russell County Medical Center, Lebanon, Virginia, paid the civil penalty in the amount of $3,750. The action was based on the deliberate administration of excessive patient doses of licensed radioactive material by the licensee's nuclear medicine technologist for a four year period, the deliberate falsifi-cation of records to reflect the prescribed doses rather than the actual assayed and subsequently administered doses, and the failure to report the misadministrations caused by this practice when it became known to the licensee's Director of Radiology.

I Office of Consolidation Items of Interest Week Ending April 27, 1990 Second Building Status On April 26, 1990, Office of Consolidation staff met'at GSA with the GSA project manager, contract negotiator, and attorney to discuss WFN Partner-ship's proposed lease addendum incorporating changes agreed to between GSA and WFN over the past seven months.

It became clear in the meeting that both GSA and NRC are concerned about the proposed wording of the lease addendum prepared by WFN.

It contains new provisions not agreed to in negotiations and refers to Exhibits that would be incorporated but are not attached to the copy forwarded by WFN. A telephone conference was held with the Partnership attorney and the missing Exhibits were requested by GSA.

GSA, NRC and WFN agreed to meet at WFN's offices on May 7,1990, to discuss GSA/NRC concerns about the lease addendum wording as proposed by WFN.

OWFN Cafeteria On April 25, 1990, Sigal Construction Com)any conducted a negative pressure test of the kitchen exhaust duct. The ex1aust duct, which.is 18 gauge black iron square duct, welded in sections, runs vertically from the secono floor slab through the Penthouse #2 slab, a distance of 210 feet.

The test, which was required by GSA, measured the negative static air pressure applied inside the duct, and the rate of leakage in cubic feet per minute, over a 30-minute period.

It also tested the duct's ability to withstand the physical strain of the highest amount of negative pressure likely to occur in j

the duct in actual use.

The test was successful. The leakage rate over the 30 minute period was so small as to be almost imperceptible on the manometer gauge and well within l

the leakage limits set by GSA.

A visual test of the fire-rated shaft enclosing the exhaust duct was performed on April 25 by Tower Construction Company. The purpose of the visual test was to verify that the shaft walls are made of fire-rated material which is at least six inches away from the duct, and the shaft has no penetrations (other than fire-rated access doors) between the second floor slab and the Penthouse #2 slab. Visual inspection confirms that the shaft is built as designed.

It has a 6" x 12" 3enetration at the fourth floor level and smaller penetrations on some otler floors where electrical conduits or communications cables pass through which will require filling and sealing.

Safety Light Corporation /USR Industries On April 19, 1990,-the Atomic Safety and Licensing Board (ASLB) held a status meeting relative to USR's request for a hearing on the NRC's Orders dated March 16 and August 21,1989. The ASLB directed that the NRC staff and USR conclude its current settlement negotiations by May 4, 1990, or be

)repared to begin the hearing the week of June 25.

If a hearing is to be-1 eld, depositions will be taken between May 7 and May 18, and written 11, 1990. On April 24, 1990 testimony will be due to the ASLB by June theAtomicSafetyandLicensingAppealBoardaffirmedtheASLB'sdecisIon that the NRC'does have jurisdiction over USR. USR may now either accept or ask the Commission to review the Appeal Board's decision.

Seabrook At 7:00 a.m., April 25, the reactor was in Mode 1 at 8% power, with preparations being made for turbine torsional testing.

On April 20, the Bankruptcy Court approved the Northeast Utilities Service Company (NUSCO)asthemanagingagentofPublicServiceofNewHampshire (PSNH) until NUSCO's buyout of PSNH.is completed. The Court further installed a new Board of Directors for PSNH, effective April 30, 1990.

NUSCO will establish management control sometime after May 1.

The ruling reportedly does not directly affect NHY management of Seabrook in that control remains from the Joint Owner's Group through NHY management.

On April 24 NHY plan tested the 94 sirens in the 17 New Hampshire communities in the 10 mile Seabrook EPZ.

NHY emergency planning management publicized the testing.

3.

Calvert Cliffs Unit 1 After a period of plant operation beginning April 13, 1990, Calvert Cliffs Unit 1 entered hot shutdown at 5:30 a.m. on April 23, 1990. The plant is progressing to cold shutdown for entrance into a scheduled 4

maintenance outage. NRC round-the-clock inspection coverage of the operational period has been suspended as of April 24, 1990, d

-Projects Branch Chief, was at the Carolina. Power and Light Company's Harris and Brunswick Nuclear Power Stations for site familiarization.

Department of Energy - Savannah River On April 25, the Regional Administrator, and selected staff members met with DOE senior management at the Savannah River site to discuss matters-of mutual interest, i.e., communications and coordination during emer-gencies and/or incidents in the area of emergency preparedness and public information. A site tour was also conducted in conjunction with this meeting.

Duke Power Company 1

On April 25, representatives of Duke Power Company were in the Region 11 l

Office to attend an Enforcement Conference to discuss a March 20. 1990

overpressurization event at their Catawba Nuclear Station.-

General Electric Company's Management Meeting General Electric's Nuclear Fuel and Components Manufacturing Wilmington Plant Manager, W. Lewis and C. Vaughan, Manager of Regulatory Compliance, were in the Region II office at 1:00 p.m., on April 26, to brief the Regional Administrator and selected RII and NMSS staff members on changes /

improvements made and planned at the facility. The meeting included discussion of plant staffing, changes in operational procedures, and I

revisions of the radiation protection program which were largely the result of a recent management audit of the Wilmington. facility. The licensee has reviewed the recommendations made by the auditor and has initiated organizational and operational changes to correct the weaknesses noted.

Virginia Electric and Power Company's Management Changes Effective May 1, 1990, the Assistant Station Manager-Nuclear Licensing and Safety at Surry has been promoted to the position of Manager of Nuclear-Engineering at VEPCO's corporate office.

In this position, he will report directly to the Vice President-Nuclear Engineering Services. A former Surry Quality Assurance Manager is assuming the duties of.the Assistant Station Manager-Nuclear Licensing and Safety.

Region III Items of Interest Week Ending April 27, 1990 1.

Fermi 2 Nuclear Plant T. O. Martin, Deputy Director, Division of Reactor Safety, and other regional representatives attended a meeting on April 24, 1990, in Headquarters with Detroit Edison Company on a concern about cracking of small-bore vent and drain line connections to the RHR system at the Fermi Nuclear Plant. The licensee has experienced cracking in several of these connections since 1987 and is undertaking a program to cut out and repair 14 of these lines in the next two weeks.

Braidwood Nuclear Power Stations I

On April 26, 1990, Dr. Carl J. Paperiello, Deputy Regional Administrator, William L. Forney, Deputy Director Division of Reactor Projects, and members of the Region III and NRR staffs met with the licensee management to present the SALP 9 report. The licensee received two SALP 1 ratings, three SALP 2 improving ratings, and two SALP 2 ratings in the seven functional areas evaluated.

Clinton Nuclear Power Station 1:

The Clinton Power Station was taken critical on April 22, 1990, after a licensee management directed shutdown to implement corrective actions following the improper rod withdrawal event of April 11, 1990. Region III i

and NRR provided augmented control room coverage from restart until the plant reached 40% power on April 25, 1990. The augmented coverage indicated that the licensee was operating the plant in a conservative manner. The Special Inspection Team will exit with the licensee on 1

April 27, 1990.

LaSalle A special team inspection was conducted April 22-27,1990, at LaSalle to I

evaluate the effectiveness of licensee ALARA actions. The team consisted of Region III and Region I inspectors as well as representatives from Brookhaven National Laboratory and NRR. A representative of the Finnish Center for Radiation and Nuclear Safety, who is temporarily working at NRR, observed the inspection. The inspection was conducted at LaSalle because of recent high cumulative worker radiation doses at the station.

Monticello Region III staff met with Northern States Power Company representatives on April 25, 1990 in the Region III office to discuss security program improvements implemented and planned at the Monticello plant. These improvements are being taken to address previously identified poor performance and to address equipment failures. The licensee's progress appears to be adequate and the previously identified problems have not been recurring.

St. Mary Medical Center and Porter Memoria1' Hospital On April 23, 1990, A. Bert Davis, Regional Administrator, James Lieberman, Director, Office of Enforcement and other metabers of Region III and Headquarters staff (from NHSS and OGC) held an Enforcement Conference with representatives from St. Mary Medical Center. A similar Enforcement Conference was held by A. Bert Davis and other NRC staff with representatives from Porter Mcnorial Hospital on April 25, 1990..The conferences were held to discuss findings from recent NRC inspections of the brachytherapy treatment programs at both facilities and to obtain additional information needed for input into NRC's decision on whether the programs.should remain suspended.

7.

Combustion Engineering A special team inspection is being conducted April 23-27, 1990 at the Com)ustion Engineering facility in Hematite Missouri by representatives from Region III, HMSS and the Occupational Safety and Health Administration.

The team's purpose is to review the non-radiological and radiological safety aspects of Combustion Engineering's activities, which include manufacturing fuel pellets for nuclear reactors from uranium hexafloride (UF6). The team will also followup on Combustion Engineering's programs implemented as a result of the uranium release that occurred August 1989-at the facility.

Arkansas, on April 23, 1990. The Chairman toured the' Arkansas Nuclear One site in Russellville, Arkansas, on April 24, and returned to Little Rock for a visit with Governor Clayton on April 25.

Arkansas Power & Light Company l

The Regional Administrator, Region IV, and NRR personnel met at the Arkansas Nuclear One (ANO) plant site with management of Arkansas Power and Light Company (AP&L) on April 24, 1990, to discuss plant performance.

4 The discussion included the status of the implementation of the AP&L Business Plan which was developed in response to the Diagnostic Evaluation Team inspection and the ANO Salp Report.

URF0 meeting with Exploration of Uranium Resources. Inc.

The Vice President, Exploration of Uranium Resources, Inc., met with the Director, URF0 in the Region IV office on April 24, 1990, to discuss recent proposals for modification of the license. The licensee operated a research and development in-situ leaching operation in Wyoming _

until approximately 1983. The facility is now undergoing restoration-t 4.

Hioh Mountain Inspection Services, Inc.

A Region IV inspector reviewed radiography activities conducted by an NRC l

licensee at a refinery in Casper, Wyoming, on April 18, 1990, at 4:30 a.m.

With the assistance of the refinery o>erator, the inspector was able to observe these activities covertly witicut the knowledge of the licensee.

High Mountain Inspection Service, Inc. The inspector identified several violations, including the radiographer's failure to perform any surveys of the ex)osure device or the guide tube after each of the two observed radiograpiic source exposures.

Fort Calhoun Station On April 21, 1990, theFortCalhounStation(FCS)wasplacingthereactor vessel head onto the reactor vessel in preparation for the conclusion of a refueling outage that began February 17, 1990. At approximately 4 p.m.

the reactor vessel head experienced an unexpected 6-to 8-inch downward drift. As a result, the two guide pins were deformed but no apparent damage to the reactor vessel or vessel internals was identified. The licensee is investigating the cause and has asked the crane vendors, Harnischfeger and General Electric, for assistance. This event has extended the licensee's refueling outage by approximately 4 days.

Region IV is reviewing the licensee's corrective action prior to the planned restart of FCS on May 12, 1990.

South Texas Project The Deputy Regional Administrator, Director for Division of Reactor Projects, and other regionel staff met with licensee management at the South Texas Project site on April 25, 1990 to discuss the current SALP report. FollowingtheSAlrmeeting,aninformationmeetingwasheld with local officials.

Uranium Recovery Field Office The Director, Deputy Director and selected staff of the Uranium Recovery Field Office met with-representatives of. Union Pacific Resources on April 26, 1990, to discuss their ground-water corrective action program for the deconsnissioned Bear Creek Uranium Mill in the South Powder River Basin of Wyoming.

Items discussed were design of evaporation systems, treatment of contaminated ground water and required water quality standards for water that may be released from the site.

Drummond. Montana On April 26, the Montana Department of Highways reported that a Troxler Model 3411 portable gauge was run over by a front end loader at a constructior. site at Drummond, Montana, about 60 miles west of Helena.

The incident had occurred on the previous day. Although the gauge was damaged..the two sealed sources in the device were apparently not ruptured. The gauge houses a 10-millicurie cesium-137 source and a 50-mil 11 curie americium-241 source._ The licensee surveyed the device and surroundings at the scene with a GM meter and did not detect contamination or unusual readings. The licensee then transported the device in its storage case to Helena. The device will be held in storage until wipe samples are analyzed to determine if there was any source leakage.

Certain of the licensee's personnel have the requisite training and equipment to analyze leak test samples and are authorized to do so by license.

Region IV will await the results of.the licensee's report of leak tests and-30-day report, and if results are negative, will review the circumstances surrounding the incident during a future inspection.

Southern California Edison Company (San Onofre)

Region V and NRR management met with Southern California Edison Company management on April 25, 1990 to review the results of the latest Systematic Assessment of Licensee Performance (SALP). The licensee did not question the SALP results, which assessed the licensee Category-1 in four functional areas and Category 2 in three areas.

Rancho Seco Nuclear Generating Station 1.

On April 26, 1990, SMUD announced the selection of Mr. David Freeman as the new General Manager. Mr. Freeman's appointment will be effective June 4, 1990. Mr. Freeman was formerly.the Chairman of.

TVA and most rectntly was General Manager of the Lower Colorado River Authority, a Texas-based water and power utility.

The petition to eliminate SMUD continues to be circulated for signatures. 27,500 signatures are necessary to qualify the petition to be placed on the November ballot.
