} D - a o.: g i w y = t HIGHLIGHTS OF WEEKLY INFORMATION REPORT ~ Week Ending May 4,1990 - Seabrook Unit'1 i - During the startup test programs for the GE steam turbine generstor torsional-test, vibrational-loads potentially exceeded the.GE specification limits. The increased vibration was experienced at the 26th harmonic of the shaft resonant . frequency, and is believed to indicate possible vibration levels which would ~ be beyond.GE' specifications for levels resulting from electrical forces =,. (negative sequence current)-once the turbine-generator's tied-to the grid. Startup testing has been curtailed while PSNH and GE assess what modifications to the LP turbines will be necessary. Consideration of the potential-for fatigue induced failure of LP turbine blading is to be included in the assessment. The plant is expected to be shutdown from four to six weeks. L The NRC Region I shift inspection coverage has been curtailed for the shutdown period. Vermont Legislature Passes LLW Legislation . The Vermont Legislature approved legislation which creates a LLW Authority and D -provides for a mechanism for the siting and development of a LLW disposal facility in Vermont. The legislation also directs the' examination of the Vermont Yankee site as a potential-host site. The Legislature did not approve 4 a: contract with the Rocky Mountain LLW Board-for disposal at Beatty through 1992. Thus generators will be required to store LLW on-site, as they have h since 1989. Governor.Kunin is expected to sign the legislation. The State-l then. intends to submit a Governor's certification-on the basis of this-legis-lation. The Legislature also adopted a bill which freezes the definition-of LLW.as of' January 1, 1989.and declares that waste'and materials covered under - this definition must be treated,. recycled, stored'or disposed in a licensed is deregulated - facility even if that material or waste, since January 1, 1989, federal agencies.. - or declared exempt from regulatory control by-the NRC or other. L, The. Governor is expected to sign this legislation as well. Babcock & Wilcox, Naval Nuclear Fuel Division On April 30, 1990, Babcock & Wilcox (B&W) personnel met with EDO, 0GC, NMSS, and Region 11 staff to describe B&W's plans for expanding its components manufacturing.. business for U.S. Navy nuclear programs. This expansion will involvezlicensing issues; i.e.., increased possession limits, transportation issues, additional vaults, a core storage facility, and increased fuel component storage. In addition, B&W discussed a super compactor, relocation of the laundry facility, changes to scrap recovery, closure of ponds, and-i- feedstock conversion. L m ..e. +
F N i. _. 4 Office of Nuclear Reactor Regulation Items of Interest Week Ending May 4, 1990 j FitzPatrick Nuclear Power Plant 1 On Monday, April 29, 1990, the NRC was informed by the Power Authority of the State of New York (the licensee) that an indication had been found while performing Inservice Inspections of the reactor vessel head at the FitzPatrick plant. The indication, located,in the dollar plate weld of the head, is i approximately one inch long on_the inside surface and is visible to the naked eye. The sub. surface length and maximum depth as determined by ultrasonic examination are.about 12 inches and 2 inches respectively, which represent . approx ma e y 50 percent through-wall. tl determine the cracking mechanism. Further analysis and welding repairs are- ., ~ being discussed. The dollar plate is located at the top of the vessel head a and is shaped like a large washer. NRC Information Notice 90-29, dated April 30, 1990, described a potential (! problem related to cracks in reactor vessel heads which are clad on the .l inside. period.- -Turkey Point, Units 3&4 Florida; Power and Light Co. (FPL) notified the NRC staff on May 2,1990 that s the Plant Manager, Jim Cross, is leaving the company, effective May '25. Mr. Cross-has accepted a position at the Trojan plant. Mr. William Pearce, the Operations Superintendent at Turkey Point, will assume the Plant Manager's duties in an acting capacity. Calvert Cliffs Units 1 and 2 Unit 1 commenced shutdown, subsequent to a short period of operation, on April'22, 1990, for a scheduled maintenance / surveillance outage. The ENCLOSURE A MAY'4, 1990-
w ; g w current schedule for restart of the unit is July 12, 1990. Items impacting the_ schedule are completion of LLRTs, manpower restraints on reactor coolant -pump seal replacements, electrical penetration inspection / repair work, and replacement of tne AFW control valves. The licensee has designated alternate weekends as non-work weekends during the outage. As previously noted, the State of Maryland has filed a petition to intervene in the licensing of-an Independent Spent Fuel Storage iacility (ISFSF) to be -located on the Calvert Cliffs site. -The State has indicated it will withdraw the petition if_the State, NRC, and BG&E can agree to procedures to be followed during the licensing process to assure any specific concerns identified by the
- State would be considered. All parties are reviewing a draft of the proposed procedures provided'by the State.
Plant Status as of 05/02/90: Unit 1: . The Unit is shutdown and has comenced the scheduled outage. The major items to be accomplished during the outage include eddy current testing 'of the~ steam generators, reactor coolant pump seal replacements, saltwater header modification and low pressure safety injection. seal modifications. Current scheduled restart is July 12, 1990. Unit 2: Cold Shutdown, Defueled - Extended refueling outage since 03/18/89. The licensee completed evaluating the.cause of cracks _in the pressurizer heater sleeves and stated that they resulted from-primary water stress corrosion which was initiated by excessive reaming'of the sleeves following pressurizer fabrication. B&W and BG&E have developed a repair plan which has been reviewed by NRR and. Region I.. Repairs to the pressurizer are expected to be completed with plant restarting near the end of the year. Harris Wakesouth Regional Airport - Update Comments from the staff review of CP&L's Hazards Study of the Wakesouth Regional; Airport were forwarded to CP&L by letter on April 30, 1990. Our review had concluded that the proposed airport met the acceptance criteria of = the Standard Review Plan for the 10 year operations projection, but future expansion in frequency of flights and heaviar aircraft could have a -significant impact on the continued safe operation of Harris. Therefore, CP&L would be expected to assess the impact of projected changes and update the FSAR when such projections significantly exceed those described in the present study. There is congressional interest in our actions on this matter. Division of Systems Technology Haddam Neck Reload Connecticut Yankee has requested technical specification changes for Haddam Neck which would allow them to operated with lower end cap pin hold defects ENCLOSURE A MAY 0 41990

In addition, -based on statistical analysis of data on inspected fuel rods, they estimate that'471 stainless steel fuel rods have through wall defects that could lead .to tladding. failures in a ant of an anticipated overpressurization transient. Their. analysis, based in part on the assumption that leakers which fail during the next operating cycle will have the same fission gas leakage characteristics: , as debris caused leakers from.the previous cycle, indicate that apprcpriate - Part 20 and Part 100. limits will not be exceeded. The licensee has completed inspection and reconstitution.of fuel in preparation 'l for the next reload and will not' reinsert any known leakers. The~ staff has concluded that the proposal is a violation of'GDC 10, which would not permit inclusion of any fuel rods which could not withstand.cntici-pated transient events without exceeding fuel design-limits. The licensee agrees with the staff conclusion, but intends to request an exemption. .The licensee can no longer obtain new stainless steel fuel, since it' advised t the fuel supplier that no additional' purchases are intended. The staff is . exploring whether engineering analysis can be developed that demonstrates the plant can be operated safely and provides justification for exemption from the t . regulations. _a Division of Reactor Inspection and Safeguards Draft Generic Letter On Procurement / Dedication On April'27, 1990, the staff forwarded a draft Generic Letter on commercial grade procurement,and dedication to NUMARC for their review and comment. A copy has also been provided to the Public Document Room. The draft Generic Letter discusses the. decision and basis for the 1-year pause in NRC programmatic l irapections inLthis area, and provides NRC views on dedication programs, critical characteristics, and corrective action (look-back) programs. The H draft Generic Letter, as proposed, is based on the assumption that the NUMARC i Working Group initiatives for improved procurement / dedication programs will be i implemented. Divisioc of Radiation Protection and Emergency Preparedness Emergency. Preparedness Workshop The Director, Division of Radiation Protection and Emergency Preparedness, NRR, and members of his staff, will conduct a one-day workshop on May 3, 1989, with Dennis Kwiatkowski, Assistant Director, Office of Natural and Technical -Hazards Programs, FEMA, and members of his staff at the FEMA Emergency Training l Center, in Emitsburg, MD. Staff of the. International Safeguards Branch will be actively involved in the pre-inventory, inventory-taking and IAEA verification phases of the inventories at both facilities. 3 .3 Domestic -Information Notice i NRC Information Notice No. 90-27 was issued on April 30, 1990. Its title was " Clarification of the Requirements for Packaging of Uranium Hexafluoride for Transportation." It is intended to aid the shippers of UF6 in achieving ) a better understanding of recent rulemaking actions completed by the U.S. l Department of Transportation which prescribe specific requirements for all UF6 cylinders and for the protective overpacks used'to ship cylinders of fissile UF6.. Transportation l Meeting on Trupact II Fabrication 1 On.May 1, 1990, Transportation Branch staff met with Nuclear Packaging, .Inc., (NUPAC), Westinghouse, and the Department of Energy (DOE) to discuss fabrication of Trupact 11 shipping containers. Specific items of discussion. were tolerances for the thickness of the container shells, weld finish, and the fabrication of the transition regions where.shell thickness changes. t ,NUPAC stated that all areas of fabrication would comply with applicable ASME-Code requirements. Dry Storage i - Public Service; Company of Colorado (PSC) has-informed NRC staff that it is reinstating design development efforts for a dry storage independent spent fuel storage installation to be located at the Fort St. Vrain (FSV) site. PSC does not expect a significant delay in submitting its application. The primary focus of this meeting was on the QA programs of the' DOE waste glass form producers. Progress was made at this meeting toward final resolution-of'DHLWM's concern regarding DOE's proposed milestone of NRC acceptance of-DOE QA programs by September 1990. Steyer, RES 492-3824 Division of Engineering NRC Low Level Waste (LLW) Research Progry The Waste Management Branch, RES, conducted an integrated review'of the NRC LLW research program on April 24-25, 1990 at the Congressional Days Inn, Rockville. 1 'The meeting was attended by pecpic representing NRC contractors, State organizations including regulators as well as developers, other government agencies including USGS, DOE, and EPA, DOE laboratories involved with the DOE remediation program, AECL, Nuclear Waste News, EPRI, HMSS and RES staff. l The NRC LLW research program is-described in the recently published NUREG-1380-(" Low-Level Radioactive Waste Research Program Plan") and its availability and Solicitation of comments were announced in the Federal Register on December 27, 1989. It was-the intent of the program review meeting to assess the breadth of the existing NRC LLW research program, to determine further research needs, and to factor this into a planned updating of NUREG 1380 in the Fall of 1990. l The overall. reaction from attendees at the program review meeting was most-favorable. Many attendees were impressed by the scope of work for the size of the budget allocated to LLW research. The major concern from States is the E necd for information in the next two years because of legislative mandates from u the LLW-Policy Act (1981) and LLW Policy Amendments Act (1985) which require each State or compact of States to have the capability of disposing of LLW i generated within its borders by the end of 1992. Input from States and the other. program review attendees along with an NMSS revised LLW user need letter .(scheduled for June 1990) will also be' factored into the updating of NUREG-1380.. In addition to the program review meeting the Waste Management Branch met with l-the U.S. Geological Survey (USGS) in ' lute April and a means for interagency cooperation on LLW research is being worked out. A 4 7 .y 3 1 Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending May 4, 1990-t Division of Safety Programs (DSP) Engineering Evaluation Report AE0D/E90-04 on " Swelling and Cracking in Hafnium Control Rods" was issued. ~ Hafnium was introduced as a neutron absorber for use in control rods in 1980. .It was meant'to be used in addition to carbon tetraboride as a replacement for-4 silver-indium-cadmium. Hafnium control rods were introduced as original equipment.in'several.later pressurized water reactors. They.have experienced swelling and cracking-as determined by several licensees using eddy current ~
- testing. Of-the 14 plants licensed ~to use hafnium rods,.six have replaced or will replaceithem, two never installed them, four have recently or may soon 9
install them,.and two are continuing to use them. Analyses by Westinghouse of the effects of swelling of control rods predict that neither physical interference with insertion nor mechanical resistance which would increase rod drop time'will be-of a large enough magnitude to invalidate FSAR safety 1 analysis conclusions. Eddy current test results to date have upheld these analyses. Given the limited examinations to date, the resort suggested that it would be prudent to require-all plants licensed to use lafnium control rods to perform NDE of these rods to determine the extent, if any, of cracking, swelling,= and wear over at least several fuel cycles corresponding to the-useable life of the-hafnium rods.. The report and recommendations have been provided to NRR for appropriate action. -DSP submitted reports on the following four topics to the Nuclear Energy Agency's l L. Incident Reporting System:. 1. Potential For Loss of Shutdown Cooling While at Low Reactor Coolant Levels. L: 2. Inter-System Discharge of Reactor Coolant. 7 L 3. Loss of Off-Site Power with Multiple Equipment Failures at Dresden Unit'2. Division of Operational Assessment (00A) During the' week of April 30, 1990, installation of the Emergency Response Data System (ERDS) hardware began in the Operatioris Center. The Sitc Acceptance ' Test is scheduled to begin-on June 12. The Vogtle IIT report preparation is continuing. The ED0 briefing is scheduled for May.18 and the report is scheduled to be final May 24. The exercise included State and local government participation. Ms. Taylor acted as a government liaison at the State' Emergency,0perations Center. McGuire Local Public Officials Meeting Robert Trojanowski, Region II State' Liaison Officer, participated-in a meeting. with local public officials from around the McGuire Nuclear Station on May 1, 1990 in Cornelius, North Carolina. The purpose of-the meeting was to discuss NRC's program for regulating nuclear power plants and to encourage dialogue with local public officials. Meeting with Maine LLW Officials NRC staff from NMSS, NRR, 0GC and GPA met with representatives from the Maine Low-Level Radioactive Waste Authority on May 3, 1990 in Rockville, Maryland. h +' ' :l International Programs' Foreign Visitors On Monday Mr. Jan Nistad, Director, and Per Bystedt of the Office of Inspec- .tions, Swedish Nuclear Power Inspectorate (SKI), met with NRR staff to discuss plant-life extension, maintenance, the SALP process, pressure-vessel and-internal inspections, radiation, containment spray additive' tank removal, the reactor vessel indicating system, technical' specifications, operating limits, Lcore limiting parameters, and inspection procedures. Later in the week they attended the NRC Regulatory Inforrwtion Conference and visited the Limerick

This week a project team from the Netherlands responsible for the development and implementation of a Dutch national _ nuclear emergency response plan asso-i . ciated with the National-Program' for Nuclear Emergency Response of the Ministry of the Environment visited the U.S. to observe a full-scale nuclear power plant exercise and to~ meet with Federal and State emergency response officials. The discussions concerned procedures to implement the URENCO Phase II Agreements signed by Ambassadors from the four governments. The URENCO governments suggested that a " Security and Classification Handbook" be prepared. The U.S. was invited to participate in the URENC0 security working committee group. An Order Suspending Brachytherapy Activities and Modifying Licenses was issued on April 27, 1990 to St Mary's Medical Center, Hobart and Gary, l Indiana. The action supersedes two CAL's issued on April 2 and April 13,1990 and was based, among other things, on the lack of management control of the~ brachytherapy program; the admissions of the licensees' authorized users that they were not familiar with the misadministration reporting requirements of 10 CFR Part 35; and on the failure to follow the licensees' procedures to prevent or identify brachytherapy misadministra-tions. In addition, the licensee has identified that four brachytherapy misadministrations may have occurred which were not reported to the NRC. The Order required suspension of all brachytherapy activities at the two facilities and an audit of all records concerning brachytherpy implants of patients since the programs inception by an independent medical consultant or organization approved by the NRC. (EN90-051) 2. An Order Suspending Operations and Modifying License and a-Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $10,000 were issued on May 2,1990 to Consolidated NDE, Inc., Woodbridge, New Jersey. The Order, among other things, requires that the licensee prohibit l any individual from using radiography sources until the individual has been retrained, submits a signed statement to the licensee that he or she under- [ stands and couaits to implement NRC and~ Licensee requirements, modifies appropriate procedures to require use of rope barriers to establish l i l restricted areas, ano hires an independent consultant to assess the l Licensee's radiation protection program. p The Notice of Violation and Proposed Imposition of Civil Penalty is based on a number of violations that represent a significant' lack of attention toward a system of NRC requirements intended to protect against exposure in. excess of 10 CFR Part 20-limits. The violations include failure to: (1) maintain direct surveillance of a high radiation area, (2) adequately post radiation area and high radiation area signs, (3) adequately perform required surveys of radiographic exposure devices after_ completing radio-graphic exposures, (4) lock the source in the shielded position upon i i completion of radiographic surveys, (5) properly establish a rcstricted area boundary, and (6) failure by a radiographer to wear required dosimetry / badges. Escalation of the civil penalty was considered appro-priate because NRC identified the violations, corrective actions were not effective, and the licensee had prior notice. The action was based on the failures to: supervise activities involving licensed material, secure licensed material L from unauthorized removal, maintain radiation levels in unrestricted areas below regulatory limits, conduct surveys to evaluate the extent of radia-tion hazards present, and post NRC regulations. The violations are of concern, as in the. aggregate, they indicate a lack of communications between staff and management and lack of management involvement in the L radiation safety program and knowledge of HRC's requirements. (EA90-016) g L 2. Carolina Power and Light Company (Robinson) paid the civil penalty in the amount of $450,000. The action was based on-violation of equipment i qualification requirements of 10 CFR 50.49. l-The licensee responded to the Notice in a letter dated September 1, 1988. g L After careful consideration of the licensee's response, the staff has concluded that the violations did occur as set forth in the Notice of Violation and Proposed Imposition of Civil Penalty. However, because of the complex nature of the requirements to ensure the accuracy of instru-mentation required to function during and following a design basis accident one of the violations was withdrawn as part of the enforcement action. L Withdrawal of the violaticn did not affect the categorization of the civil penalty under the " Modified Enforcement Policy Relating to 10 CFR 50.49." l The case was discussed ~in SECY-90-083. (EA87-166)' 3. Consolidated Edison Company (Indian Point Unit 2) paid the civil penalty in.the amount of $50,000. The action was based on a violation involving a revision to the licensee's emergency plan which would have lowered the Emergency Action Level classification of certain events from what they would have been classified under the licensee's approved emergency plan. The licensee responded to the Notice in a letter dated January 25, 1990. After careful consideration of the licensee's response, the staff has concluded that the violation did occur as set forth in the Notice of Violation and Proposed Imposition of Civil penalty and the licensee did not provide an adequate basis for reduction cf the Severity Level or mitigation of the civil penalty. On April 25, 1990, Region I conducted an inspection of a field site in East Vineland, New Jersey, where radiography was being performed by Consolidated NDE, Inc. personnel. Numerous violations of NRC requirements were identified. The licensee was already under a Confirmatory Action Letter from a previous inspection conducted in Lacey Township, New Jersey, on March 20, 1990'during which the NRC had observed similar violations. ^ Because of the ineffectiveness of-licensee corrective actions to prevent recurrence of these violations, Region I issued.a second Confirmatory Action Letter on April 26, 1990, documenting the licensee's commitments to remove the two individuals responsible for the April 25 violations from radiography activities' and to meet with the NRC on April 27, 1990 to-discuss the April 25, 1990 inspection findings, their causes, and planned corrective actions. As'a result of the meeting with the licensee NRC issued an immediately' effective order to suspend operations until certain short term actions are completed and to modify the license pursuant to the development and implementation of certain long term actions. The short ' term actions-include prohibiting any individual from using radiography sources until the individual has been retrained and instructed (1) on-existing procedures and~NRC requirements, (2) on the findings and L corrective actions concerned with the March 20, 1990 and April'25, 1990 events, and (3) on the licensee's specific disciplinary. program for individuals who do not adhere to requirements. Barnett Industrial X-Ray Oak Ridge Associated Universities reported on April 30, 1990, it's l' initial results of cytogenetic studies conducted on blood samples obtained from a radiographer and radiographer's assistant exposed as a result of a radiography incident at Ardmore, Oklahoma, on April 6,1990. Oak Ridge reported whole body equivalent expasures of 17 rads (8 to 27 rads with 95 + percent confidence) for the radiographer and 24 rads (13 to 34 rads with 95percentconfidence)fortheassistant. Region IV is continuing its inspection and review of the overexposure event and is planning to interview tia radiographer and the assistant again next week. Taylor, EDO dated 4/1/90 The Commission met to discuss collegially topics of individual commissioner interest. 1. White Flint Elevators chairman Carr requested n recheck of the programming to avoid unnecessary duplication of stops. 2. Deslan Certification Reviews The Commission discussed priorities for assigning resources for design reviews and noted that the staff was scheduled to brief 9 the Commission on evolutionary LWR certification issues on April 27, 1990. Commissioner Remick expressed concern about the current allocation of resources and suggested a review of the staff's capability for conducting thorough licensing . reviews in the future. Chairman Carr noted that the staff had performed a licensing resource review at the request of former Chairman Zech. The Commission requested: a) an early report from the staff on whether part 52 is sufficiently clear on defining what constitutes an essentially complete design; b) recommendations from the staff on how to approach inspections, tests, and analyses for new designs; and c) a report on how the staff views the licensing review basis document. 3. Princioles of Good Reaulation All Commissioners favored incorporation of Commissioner Rogers' l proposed statement of the principles of good regulation (COMKR-90-001) into the mission and regulatory philosophy section of the Five Year Plan. Chairman Carr suggested minor revisions. 4. NRC Role in Utility Manacement-falated Issues l This topic was discussed without final resolution. Commissioner Curtiss expressed the view that the NRC should inform licensees about what is wrong with their operations but NRC should not be drawn into advising licensees on how to correct their problems, how to manage their operations or who should be managing their activities. Commissioner Plant Visits Commissioner Curtiss suggested consideration of additional coordination among Commissioners' offices of visits by individual Commissioner's to nuclear power plants so that most plants get at least one Commissioner visit every year. 6. Recoonition of Good Performance by Nuclear Facility Personnel Commissioner Remick discussed his proposal (COMFR-90-002) to give appropriate recognition to certain good-performers among nuclear facility personnel. 7. Suquested Toolcs for Next SES Conference Chairman Carr invited the Commissioners to suggest topics to him for inclusion in the agenda for the next SES conference. g 8. Commission Meetinas The Commission agreed that the current level of meeting activity appeared appropriate but requested further improvemen in the quality of slide presentations (more meaningful slides are needed and the direct reading of slides at meetings should be eliminated.) Commissioner Remick suggested that consideration be given to increasing the interaction of less senior staff with the Commission at Commission meetings and briefings. B.
