L. T. Vilburn License No. 34-10921-03 President 619 Oak Street Cincinnati, Ohio 45206 Gentlemen:

This refers.to the routine safety inspection conducted by Messrs.

J. R. Mullauer and J. L. Lynch of this office on March 9 and 10, 1982, of activities authorized by NRC Byproduct Material Licenses No. 34-10921-02 and No. 34-10921-03 and to the discussion of our findings with you at the conclusion of the inspection.

The inspection was an examination of activities conducted under your license as they relate to r'ediation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

' This inspection also included a measurement of the outpu' of your teletherapy equipment. Our measurement was made using a Victoreen adel 570 Condenser s

' 'R-Meter with Model 621 chamber, calibrated by the National Bureau of Standards Laboratory. The source-to-chamber distance was 80 centimeters and the field size used was ten by ten centimeters. After applying standard correction factors for temperature, pressure, sattenuation, inverse square, timer error, s

and rbentgen-to-rad conversion, our measurement of the output of your tele-

' therapy equipment was 77.3 rads per m_inute.

The most recent spot check of your teletherapy equipment, conducted on February 25, 1982, reported a nicasured output for the same exposure parameters of 80.5 rads per minute. Applying a s

correction factor to account for 12 days decay, the value calculated from the February 25, 1982, calibration becomes 80.2 rads per minute.

Based on the close agreement in measured output values, we feel the actual output of your teletherapy equipment to be within established guidelines of 15 percent.

.In addition to the above areas, the inspectors examined actions described in your letter dated September 20, 1978, regarding apparent items of non-compliance found during our August 3 and 4, 1978, inspection. We have no

's further cuestions regarding these matters.
