APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket:
50-458 River Bend, Unit 1 CPPR-145 Based on the results of an NRC inspection conducted during the period of February 22-26, 1982, and in accordance with the Interim Enforcement Policy 45 FR 66754 (October 7, 1980), the following violation was identified:
Failure to Follow Storage Procedures for Structural Steel Fasteners Criterion XIII of Appendix B to 10 CFR Part 50 requires that measures shall be established to control the handling, storage, shipping, cleaning, and preservation of material and equipment in accordance with work and inspection instructions to prevent damage or deterioration.
Stone and Webster Specification 210.310, Revision 3, "St'ructural Steel,"
states, " Fasteners may be stored outside on cribbing or pallets.
Con-tainers shall be covered with a waterproof cover so placed to allow adequate air circulation to limit condensation and prevent entrance of rain into the packaging.
Care shall be taken when packages are opened that the material is protected from weather by canvas or other means during erection."
Contrary to the above:
On February 22, 1982, the NRC inspector inspected the storage conditions of Category I t,tructural steel and fasteners.
During the inspection, the NRC inspector observed that some A-325 bolts were uncovered to the environment.
Some of the containers were opened and water had accumulated to the top of the containers.
Some of the bolts had begun to rust.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including: (1) the corrective steps which will be taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Consideration may be given to extending your response time for good cause shown.
