F FINAL REPORT SIGNIFICANT CONSTRUCTION DEFlCIENCY NO. 27 "Switchgear Breaker Position Switch Operator Defect" INTRODUCTION This report is submitted pursuant to 10CFR50.55(e) (3).
It describes a deficiency in the 4.16 KV Class lE Switchgear as modified and installed and is related to Significant Construction Deficiency Report No. 11 titled "Switchgear Breaker Posi-tion Switch Operator Defect."
DESCRIPTION On March 13, 1981, Louisiana Power & Light Company's Startup Organization advised Ebasco Services, Inc., of a problem with the as-installed breaker position switch operator mechanisms.
This deficiency is documented on Nonconformance Report No.
W 3-2574.
The extent of the deficiency is as follows: The breaker position switch operators have been modified in accordance with Significant Construction Deficiency No. 11 and installed under General Electric's supervision.
During subsequent installa-tion of the breakers, an interference was detected between the operator guide and the breaker are chute covers which prevents breaker insertion in the gear.
This deficiency involved thirty-three (33) breaker position switch operators.
SAFETY IMPLICATIONS The as-installed mcdified breaker position switch operators with the added guides prevent the breakers from being inserted onto their respective location.
There-fore, this deficiency if left uncorrected could result in an inadequate installa-tion and is considered to be a reportabic item under 10CFR50.55(e).
CORRECTIVE ACTION Corrective action involved sliding the position switch operator guides horizontally to align with the 6.9 and 4.16 KV breakers in accordance with General Electric Drawing No. 0269AZ835.
This work has been completed by LP&L under the start-up system pre-op testing program with General Electric providing technical direction.
Fourteen (14) of the breaker position switch operators affected are located in switchgear 3B3-S, another fourteen (14) are in 3A3-S, and another five (5) in 3AB3-S.
All thirty-three (33) safety-related breaker position switch operators affected have been adjusted.
