APPENDIX A NOTICE OF VIOLATION Jaca and Sierra Testing License No. 52-19064-01 Laboratories, Inc.
As a result of the inspection conducted on January 13, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified.
License Condition 12 requires that licensed material shall be used by or under the supervision and in the physical presence of Pablo E. Agosto.
Contrary to the above, licensed material has been used by two of your employees on numerous occasions during 1981 when Pablo E. Agosto was not present.
This is a Severity Level V Violation (Supplement VII).
l Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this of fice within thirty days of the date of this Notice, a written statement or explanation m reply, including: (1) admission or denial of the alleged viola-tien; (2) the reasons for the violation if acmitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achleved. Consideration may be given to extending your response time for good cause shown.
