l License Nos. DPR-26 AUG 2 51977 DPR-64 APPENDIX A NOTICE OF VIOLATION Based on the results of an NRC inspection conducted on August 2-5, 1977, it appears that certain of your activities were not conducted in full compliance with the conditions of your license as indicated below.

These items are characterized as Deficiencies.

A.

Technical Specification 6.8.2 states in partn> Each procedure and

. administrative policy...and any changes to them shall be reviewed and approved for implementation in accordance with a written ad-d ministrative control procedure."

SAO-100, Revision 4, July 1,1975, Station Administrative Order Policy, paragraph 2.5 states in part: "All Station Administrative Orders shall be reviewed biennially...," and paragraph 3.0 requires the Plant Manager and certain other plant personnel to... report biennially by written memo to the Manager, Nuclear Power Generation on the results of their review of SA0's."

I SAO-101, Revision 4, January 15, 1976, NPG Management Policy, the l

paragraph entitled " Reports Required" in reference to Administrative i

Directives, requires that the first report of biennial review is due no later than one year from the date of this order which is January 19, 1976.

j Contrary to the above, from July 1, 1975 to August 5, 1977, nine of l

approximately thirty SA0's, became due for periodic review and there was no documentation of the results of periodic reviews.

In addition, from January 19, 1977 to August 5,1977, six of seven Operation Administrative Directives became due for periodic review and there was no documentation of the results of periodic reviews.

B.

Technical Specification 6.8.1 states in part: " Written procedures and administrative policies shall be established, implemented and maintained...."

2 GAD-7, Revision 0, December 8, 1976, Procedure Control, paragraph 3.0.2 (with respect to 10 CFR 50, Appendix B, Criterion VI, which requires that measures be established to control the issuance of documents, such as procedures) states:

The operations staff will maintain an index of procedures identifying the latest revision and temporary change."

~ Contrary to the above, on August 2,1977, an index of procedures (covered by QAD-7) identifying the latest revision and temporary change had not been established, and on August 4,1977, a completed index for procedures upgraded as of August 1,1977, did not contain the latest temporary changes to these procedures.
