OPR-5 DPR-26 DPR-64 APPENDIX A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted August 30 - September 2, and September 6-8, 1977, it appears that one your activities was not conducted in full compliance with NRC regulations and the conditions of your licensed as indicated below. This is a deficiency.

Unit 2 and 3 Technical Specifications 6.11 and Unit 1 Technical Specification 3.2.6 require that procedures for personnel radiation protection be prepared consistent with the requirements of 10 CFR 20 and be approved, maintained, and adhered to for all operations involving personnel radiation exposure.
General Administrative Directive RS-GAD-2, Revision 1, " Radio-logical Health and Safety Procedures," dated February 24, 1975, developed pursuant to Unit 2 Technical Specification 3.2.6 requires that all employees obey certain general radiation safety rules including 7.1.18 which states "Do not use or otherwise improperly handle any other individual's exposure monitoring device."
Contrary to the above requirement, on September 6, 1977, an indi-vidual used the exposure monitoring device (film badge) of another individual when he made an entry into the controlled area.
