Description:
The HPCI stop valve main disc flange which shou die attached to the main disc by four cap screws, was found with three of the cap screws broken This flange is a ported unit intended to function in concert with an internal pilot valve to ensure stable opening of the turbine stop valve.
Cause and Corrective Action:
This problem was discovered while maintenance was being performed on the pressure adjustment. mechanism for the Stop Valve's steam balance chamber per SIL No. 352. The four cap screws have been retrieved for ex-amination, and a request for engineering planning and support submitted to as-certain the reason for this event.
