10CFR50, Appendix B, Criterion,V and ANSI N45.2-6 require that quanti-tative criteria, such as dimensions and tolerances, shall be specified, as appropriate, for determining satisfactory work performance and quality compliance.
Contrary to the above, the Control Room Ventilation and Air Conditioning System was joined together by bolting without specified controls for hole sizes, tolerances in the job specification, work instructions, drawings, and/or standards.
Nhis is a Severity Level IV Violation.

For bolt holes which exceed the specified bolt diameter by
. greater than 1/16 inch, suitably sized washers shall be
'provided to accomplish proper bearing surface. This corrective action has been provided in dispositioning DLC-SQC NSD 1905.
'i 1.2 For all installed 5/16 inch QA Category I duct _ connections,.

each bolt will be removed and replaced with a new bolt, nut, and washers.
1.3 Specification 2PVS-935 will be revised to include the following statements.
"For all installed 5/16 inch QA Category I duct connections, each bolt will be removed and replaced with a new bolt, nut, and washers. For all future 5/16 inch bolted connections, washers shall be required. Vendor shall be instructed to use 3/8 inch punch."
2.0 Corrective Action taken to avoid future Violations 2.1 To prevent future violations from taking place on QA Category I duct connections, the specification 2BVS-935 will be revised as above.
In addition, the appropriate Inspection Plan will be revised to assure that the assembly meets the above-stated criterion.
3.0 Expected Completion 3.1 The corrective action for the presently installed duct con-nection shall be completed by January, 1984.
