RESPONSE TO NOTICE OF VIOLATION IE INSPECTION 81-32 Finding:

As a result of an inspection conducted on November 17, 1981 to January 8, 1982, and in accordance with the Interim _ Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified.

10 CFR 50, Appendix B, Criterion V, states that activities affecting quality shall be prescribed by documented instruction and shall be accomplished in accordance with these instructions. Maintenance procedure 1410-Y-26, paragraph 6.3.1.c and Welding and Cutting Permit, item 2D state "All floor and wall openings within 40 feet of the operation will be covered tightly".

Contrary to this requirement, on December 16, 1981, a floor pipe penetration within 40 feet of a welding operation was not covered. Hot weld slag material dropped through the opening and ignited a cloth laying 'on a cable tray below the penetration.

This is a Severity Level IV violation (Supplement 1).

Response

Initial Corrective Steps Which Have Been Taken and Results Achieved:

Contrary to NRC Inspection Report 50-289/81-32 item 5.c, the two craftsmen responsible for the fire were not immediately terminated for their failure to follow procedure. The craftsmen involved in this incident were transferred from TMI-1 to TMI-2 for unrelated reasons in late December 1981. Contractor supervision conducted an interview wherein the subject craf tsmen were re-instructed in the requirements of procedure 1410-Y-26. The craftsmen

reported the unavailability of flameproof cloth covers from the Contrator's tool room as. contributing to their failure to properly seal floor openings at the welding location.

An adequate supply of this material was available in the Maintenance Depart-ment tool room, of which the craftsmen were unaware. The Contractor's tool room was immediately stocked with additional flame proof cloths.

Corrective Steps Taken To Avoid Further Violation:

As a topic of discussion in their weekly safetymeetings with Foremen of -

all craf ts, Contractor supervision has re-instructed their work force'in the requirements and importance of compliance with procedure 1410-Y-26.

In addition, Fire Protection Engineering has increased their surveillance of welding and burning activities / procedure compliance.

Date of Full Compliance:

Full Compliance has been achieved. However, Fire Protection Engineering will report the results of their surveillance activities by March 31, 1982 to management personnel responsible for Contractor performance.

Should any further corrective actions be deemed necessary, they will be implemented in a timely fashion.
