Dear Mr. Collins:
This supplemental response is hereby submitted as a result of discussions with Mr. Dickerson and members of the PSC staff wherein Mr. Dickerson stated that Region IV desired an additional response to Items 1 and 3 contained in the Notice of Violation as a result of I&E Inspection 81-26.
Public Service Company was requested to review their responses to Items 1 and 3 with specific reference to generic issues that might be involved.
PSC's response to I&E Inspection 81-26 (P-82022, attached) was reviewed anc the following supplemental response is hereby submitted:
ITEM 1 There is, in some cases, a possibility that a clearance could be issued that would constitute a violation of a Technical Specification L.C.0.'s unless the clearance is reviewed y
carefully against the requirements of the applicable i

PSC will therefore revise the standard clearance j
format to include a highlighted section containing a list of 5,
the applicable L.C.O.'s and a signature block will be added i
to the L.C.0. section of the standard clearance form to

The clearance format changes will be complete by June, 1982.

In general, work accomplished under a Controlled Work Procedure (CWP) will contain all required signatures on the CWP cover sheet prior to placing the affected system (s) in service.
In cases where all signatures are not on the cover sheet prior to placing the system (s) in
- service, the Shift Supervisor shall obtain management approval prior to releasing the affected system (s) into service. An Operations Order pertaining to the management approval requirement will be issued by March 7, 1982.
