I & E Inspection Report 81-25

Dear Mr. Collins:

This letter is in response to the Notice of Violation received as a result of inspections conducted at this facility during the period November 16, 1981 to November 20, 1981. The specific items found in violation and responses are as follows:

1. Technical Specification 5.2.10a 3 requires that the diesel engine for the diesel driven fire pump be inspected each refueling shutdown.

Contrary to this requirement, the diesel engine for the

diesel driven fire , pump was not inspected during the May 1981 refueling shutdown. It was last inspected in September 1980 and December 1981.

2. Technical Specification 5.10.1 requires an annual actuation test of the heating and ventilation dampers and fans in the three room control complex Contrary to this requirement, this surveillance test was last performed on July 17, 1980, an interval of 16 months to the date of this NRC inspection. This interval is in excess of the +25 percent adjustment to surveillance intervals permitted by the Technical Specification definition (2.18) of surveillance intervals.

3. Technical Specifications 5.10.2 (a) and (b) require that the pressure and weight of the Halon storage cylinders be checked quarterly.

Contrary to this requirement, Halon storage cylinder weight and pressure checks were not performed between February 20, 1981 and July 1, 1981. An interval of 19 weeks. This interval is in excess of the +25 percent adjustment to surveillance intervals permitted by the Technical Specification definition (2.18) of surveillance intervals.

These items constitute a Severity Level V Violation.

Response

(1) The corrective steps which have been taken and the results achieved:

Immediately upon learning of the violations it was determined that a program be initiated to maintain a guideline as to surveillance performance time periods, and to establish control over the types of surveillances to insure completion within these time limitations. The Scheduling Department was given .

responsibility of controlling time limits on surveillances.

All involved personnel were made aware of previous Scheduling problems. This has improved the ability to implement the new Surveillance Scheduling Program.

(2) Corrective steps which will be taken to avoid further violations:

A new Surveillance Scheduling Program (Level III Procedure SSP-1) was written and is presently being reviewed for approval. The context of the procedure has been discussed by all concerned to insure that implementation can be attained and maintained. The position of the person controlling the Surveillance Scheduling Program is being upgraded to provide a more technically qualified individual. A new Surveillance Scheduling computer program is being completed by on site Computer Services personnel to maintain a better method of Surveillance Scheduling control.

The surveillances are presently being renumbered by only the suffix portion of the number. This is to aid in determining the type of test.

(3) The date when full compliance will be achieved:

Compliance specifically addressing the violations was achieved on January 4, 1982.

The compliance regarding actions, as listed, to avoid further violations will be achieved by April 1, 1982.
