Report No. 52-0Cl21-02/82-01 This refers to the routine safety inspection conducted by Mr. R. A. Brown of this office on January 12, 1932, of c.ctivities authorized by NRC License No. 52-06121-02 for the Nuclear Medicine facility.
Our preliminary findings were discussed with Dr. Conde of your staff at the conclusion of the inspection.
The inspection was an examination of the activities conducted under your. license as they relate to radiation safety and to compliance with the Commission's rules and regulations and the conditions of your license. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector.
During the inspection, it was found that certain activities under your license appear to violate NRC requirements.
This item and references to pertinent requirements are listed in the Notice of Violation enclosed herewith as Appendix A.
Elements to La included in your response are delineated in Appendix A.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure will be placed in the NRC's Public Document Room unless you notify this office, by telephone, within ten days of the date of this letter and submit written appli-cation to withhold information contained therein within thirty days of the date of this letter.
Such application must be consistent with the requirements of 2.790(b)(1).
