NOTICE OF VIOLATION St. Luke's Episcopal Hospital License No. 652-16061-01 As a result of the inspection conducted on January 14, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified.
A.
10 CFR 35.14(e) requires in part that a sealed source which has been stored and n,ot used be tested for leakage prior to any use unless it has been leak tested within six months prior to the date of use.
Contrary to the above, the 180 microcurie Cesium-137 source used as a cali-bration source was not leak tested in July 1981 when it was placed into service after having been stored for a period exceeding one year.
This is a Severity Level V Violation (Supplement VII).
B.
License Condition 17 requires that licensed material shall be used in accordance with statements, representations, and procedures contained in your application dated August 16, 1979 and letter dated July-9, 1980. One statement in your letter indicates that a high range survey meter capable of measuring over 1000 mR/hr will be available in the Nuclear Medicine Depart-ment.
Contrary to the above, as of the date of the inspection, no high range survey meter was available for use in the Nuclear Medicine Laboratory.
This is a Severity Level V Violation (Supplement VII).
C.
License Condition 17 requires that licensed material shall be used in accordance with statements, representations, and procedures contained in your application dated August 6, 1979. One procedure in your application requires daily constancy checks of your dose calibrator with records be maintained of the results.
Contrary to the above, as of the date of the inspection, no records have N,
been maintained of daily constancy checks performed on the dose calibrator.

D.
License Condition 17 requires that licensed material shall be used in accordance with statements, representations, and procedures contained rin your application dated August 16, 1979. One procedure'in your application requires records be maintained of surveys performed on radioactive waste prior to disposal as normal waste.

Contrary to the above, as of the date of the inspection, no records have been maintained of surveys performed on radioactive waste prior to disposal.
This is a Severity Level VI Violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration.rcay be given to extending your response time for good cause shown.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or af firmation.
