e Appendix NOTICE OF VIOLATION Eli Lilly and Company License No. 13-01133-02 As a result of the inspection conducted on February 25 and 26, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
License Condition No. 20 requires that licensed material be possessed and used in accordance with statements, representations, and proce-dures contained in.various dated applications and letters.
Eli Lilly radiation safety manual, page 9, submitted with application dated October 28, 1977, states that personnel performing radioiodine labeling who handle more than 10 millicuries per week will have weekly or biweekly thyroid checks.
Radiation safety manual, page 9, submitted with application dated August 3, 1978, states that individuals handling 4
500 microcuries of radioiodine for labeling or inorganic radioiodine salts must have a thyroid check every two weeks.
Contrary to these requirements, personnel handling quantities of radioiodine exceeding one or both of the above action levels, have not had thyroid checks every two weeks.
Specifically, an individual in laboratory N-22 did not have thyroid checks performed in March thru June 1979, and June thru September 1980.
This is a Severity Level V violation (Supplement VII).

Radiation safety manual, pages 33-36, submitted with application dated August 3, 1978, requires. laboratory surveys to be performed at frequencies dependant on the radioisotope, its quantity and use.

l in all applicable labs at the required frequencies. Specifically, monthly surveys were not performed in R. J. Herberg's lab (s) at your Greenfield facility between June 20, 1980 and January 7,1981, nor i
in J. R. Hutchins' lab at the McCarty Street facility between October f
31, 1980 and March 9, 1981.
This is a Severity Level V violation (Supplement VII).
