by NRC Byproduct Material License No. 12-17684-01 and to the discussion of our findings with Edgar N. Gipson, M.D. and Darryl Ellis, R.T., at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Connission's rules and regulations and with the conditions of your license. The inspection consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
During this inspection, certain of your activities appeared to be in noncompliance with NRC requirements, as specified in the enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
In addition to our concern for corrective action with respect to the J
specific items of noncompliance noted, we are concerned about your j
management control system that has pemitted these items to occur.
Consequently, in your reply to this letter, please describe those l
actions taken or planned to improve the effectiveness of your manage-ment control over your licensed activities so as to preclude the recurrence of these or similar items of noncompliance. Enclosed is Appendix B. " Management Control" to provide you with some guidance for your response.
