fl0TICE OF VIOLATION Department of the Army U. S. Army, Tripler Army Medical Center Oahu, Hawa11 96819 License flo. 53-00458-04 As a result of the inspection conducted on February 3,1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
A.
License Condition 20. states that the licensee shall possess and use licensed material in accordance with statements, representations, and procedures contained in application dated October 11, 1978; letter dated October 9,1979; and letter dated January 3,1979 (received January 18,1980).
Item 18, " Waste Disposal Procedures" of the application dated October.11, 1978 states that for licensed material with half-lives of less than 60 days, radioactive waste materials will be held for decay until monitoring with a thin-window geiger counter indicates that the difference between the waste and background count rates is not statistically significant.
Contrary to the above, at the time of the inspection, empty vials which had contained 100 nicrocuries of iodine-125 on receipt at the Drug Urinalysis Test Center, Schofield Barracks, were not monitored prior to disposal to normal trash.
This is a Severity Level IV Violation (Supplement IV).
B.
10 CFR 20.401(b) states that each licensee shall maintain records showing i
the results of surveys required by 10 CFR 20.201(b).
In addition, Iten 17. " Current Survey Frequency" of a letter dated October 9, 1979, which is referenced in License Condition 20., states that monthly contamination wipe surveys will be conducted at the Drug Lab, Schofield Barracks on a monthly basis by the Radiation Protection Office.
Contrary to the above, at the time of the inspection, no records could be located of contamination wipe surveys conducted by the Radiation Protection Office at the Drug Lab, Schofield Barracks, during June, July, or October of 1981.
This is a Severity Level VI Violation (Supplement VII).
