Dear Mr. Wiedeman:
We are in receipt of your letter dated February 4,1982. The following is submitted in response to your request.
1.
Five treatment charts will be provided for the teletherapy unit at three months in advance of the month of use. A check to see that they remain current will be accomplished by the thera-py technologist, and also checked by our physicists consultant during his monthly spot checks.
2.
The survey as required by condition 18 of our license will be conducted again and the information submitted to the appropri-ate N.R.C. board as requested. This will be accomplished by March 15, 1982.
This response is submitted under affirmation as required by Section 182 of the Atomic Energy Act of 1954.
