APPENDIX A NOTICE OF VIOLATION Fauquier Hospital License No.- 45-19484-01 i
As a result of the. inspection conducted on November 4,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following l
violations were identified.
l A.
Licens Condition 16 requires that licensed material be used in accordance with s atements and procedures contained in your application dated July 24, 1980.
One-procedure in your application requires your Medical Isotopes Committee to meet at quarterly intervals.
i Contrary to the above, your Medical Isotopes Committee has met on only one occasion since your license was issued on November 20, 1980.
Y This is a Severity Level V Violation (Supplement VII).
B.
License Condition 16 requires that licensed material be used in accordance with statements and procedures contained in your application dated July 24, 1980. One procedure in your application requires that quarterly linearity checks be performed on your dose calibrator.
i Contrary to the above, your dose calibrator's linearity has been checked on only one occasion since your license was issued on November 20, 1980.
This is a Severity Level V Violation (Supplement VII).
C.
License Condition 16 requires that licensed material be used in accordance with statements and procedures contained in your application dated July 24, 1980.
One procedure in your application requires that all areas where 4
radioactive materials are used shall be surveyed for contamination on a weekly basis.
J Contrary to the above, areas where radioactive materials are used have not been surveyed for contamination since your license was issued on November 20, 1980.
This is a Severity Level V Violation (Supplement VII).
D.
License Condition 16 requires that licensed material be used in accordance with statements and procedures contained in your application dated July 24, 1980. One procedure in your application requires that ventilation rates in your Nuclear Medicine Department be checked on a quarterly basis.

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken ar.d the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
