Dear Administrative Judges:
Enclosed is the affidavit of Mr. Dennis O'Boyle, updating his earlier testimony on channel deformation.
I have also enclosed a deviation report dated November 25, 1981 concerning an incident which occurred at Commonwealth Edison's Quad Cities Nuclear Station.
This incident has been determined to have no safety significance and is unrelated to the high density racks, the subject of this proceeding.
However, since the incident involved a bowed fuel channel, we are producing this report pursuant to the tenets of full disclosure as articulated in Duke Power Company (William B. McGuire Nuclear Station, Units 1 and 2),
ALAB-143, 6 AEC 623 (1973).
