This Command is authorized to use Tech / Ops Model 660 Gama Ray Prnjector as a storage and transport device. The following information is submitted in accordance with paragraph 71.12 (b)(1)(iii) of reference (a).
a.
The name and license number of the activity using the package is Shore Intermediate Maintenance Activity San Diego, license number 04-13252-01.
b.
The name and certificate number of person to whom the package approval has been issued is Technical Operations, Inc., Radiation Products Division, Burlington, Massachusetts 01803, Certificate Number 9033.
c.
The package identification number specified in the certificate is USA /9033/B.
2.
This Comand's Quality Assurance Program for Radioactive Material Package was given approval number 0058 by docket number 71-0058.
3.
It is requested that this Command be registered to transport licensed material in the Tech / Ops Model 660 exposure device.
