The certificate will expire on April 30, 1982. Because the certificate reference consist of an application and many supplements, we request that you submit a consolidated application, with drawings, incorporating all previous submittals. The consolidated application need not include your QA program but should continue to include quality control require-ments and operating procedures.
The consolidated application (8 copies) should be submitted at least thirty (30) days prior to the expiration date of the certificate.
In addition, the renewal fee prescribed in 10 CFR 5170.31 should be included with your request.
