99900324/81-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on December 14-18, 1981, it appears that one of your activities was not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quanti-tative or qualitative acceptence criteria for determing that important acti-vities have been satisfactorily accomplished."
Nonconformance with these requirements is as follows:
Process Spec. No. 252-1-38, Revision B, dated December 14, 1981, states in part: "If a defect or noncompliance is suspected to be reportable to the US NRC, a cooy of the attached Form No. ET-2550 shall be filled out by the department supervisor in which the component is fabricated and approved by the department manager.
Contrary to the above, Form No. ET-2550 had not been filled out for any defects or noncompliances suspected to be reportable.
[ Note: at the time of the NRC inspection the following defects were or had been suspected to be reportable:
(1) electrical penetration assemblies (EPA) with deficiently soldered connectors at WPPSS-WNP 2; (2) cable jacket cut at STP; (3) im-proper cable assembly at San Onofre; and (4) defective Rockbestos RSS-6-100 series cable used as pig-tails at STP.]
