Underrated / Unqualified Terminal Strips on Limitorque Valve Operators INTERIM REPORT 6 DATE:

Consumers Power Company Midland Plant Units 1 and 2 BecM al Job 7220 Status of Corrective Action and Investigation The status of actions in progress to resolve the subject MCAR follows.
1.
Bechtel field engineering has generated a nonconformance report (NCR-3780) against valves 2MO-1111, 2M0-1101B, and 2M0-1120B purchased from Westinghouse Electric on Purchase Order (P.O.) 7220-M-123A(Q)-AC, Items 4.2, 5.4, and 5.8, respectively. This NCR was initiated because of the presence of an unqualified terminal block, Buchanan 0824, as reported in Item 1 of MCAR 46, Interim Report 5.
Resolution of thi,s NCR is pending resolution of Item 2c below.

A meeting between Consumers Power Company, Bechtel, Henry Pratt, and Limitorque was held at the Ann Arbor office on December 11, 1981, to develop further actions in the program to inspect and-replace underrated terminal blocks for the valves supplied on P.O. 7220-M-132(Q)-AC.
The following agreements regarding ter-minal blocks were reached at the meeting.
a.
Marathon 100 Series terminal blocks installed on some Limi--
torque operators supplied in P.O. 7220-M-132 do not conform to industry and project requirements and thus all Marathon 100 i
Series terminal blocks must be replaced with Bechtel-approved terminal blocks.
b.
Limitorque stated that use of unqualified terminal type Buchanan 0824 is limited to P.O.
7220-M123A(Q)-AC with Wes-tinghouse Electric, based on their specific terminal block requirements (in accordance with Limitorque records). These blocks are not qualified or intended to be and would require replacement by Bechtel-approved terminal blocks.
It was concluded that Buchanan 0824 terminal blocks had not been identified on any other purchase order based on random in-spection results.

Limitorque stated that ceasinal blocks types Buchanan 0222, Buchanan 0524, Curtis Type L, Marathon 300 Series, and General Electric EB-5 were environmentally qualified for inside containnent use as previously reported. However, adequate documentation is not available. Limitorque anticipates com-
_pleting environmental retesting of these terminal blocks for inside containment use and submitting a report to Bechtel by April 15, 1982, instead of December 31, 1981, as previously reported.
d.
Limitorque stated that presently it has no specific plans for an environmental testing program for terminal blocks types Kulka 622 and Beau 7600 for outside containment use. These blocks have been qualified by analysis. Limitorque stated that the criteria used in its analysis for outside containment use is more stringent than the environmental parameters (for outside containment) provided by Bechtel. Limitorque could not provide a specific date by which it will provide its evaluation of the suitability of these terminal blocks. The date previously reported was December 31, 1981.
Bechtel is pursuing information concerning the terminal blocks listed above from Limitorque before proceeding to inspect and re-place the identified blocks, as necessary.
The responsibility and a schedule for replacing nonconforming e.
blocks will be established following acceptability of the terminal blocks, as specified in Items 2c and 2d, above.

Bechtel is expediting Limitorque to establish a root cause for the presence of unqualified terminal blocks Buchanan 0524, Buchanan 0824, Marathon 300 Series, Beau 7600, Kulka 622, and General Electric EB-5 used on Limitorque operators inside and outside containment.

The 17 operators (Item 6, Interim Report 5) to be inspected upon receipt from the vendors at the jobsite for the presence of non-conforming tecninal blocks was changed to 18 because an additional valve was included from Purchase Order 7220-M168(Q)-AC. The types of terminal blocks found to exist on the eight operators inspected on receipt by the resident electrical engineer are as follows. All operators are located outside containment.
