Freedom of Information Act Reouest; To whom it may concern:

Pursuant to the Freedom id Information Act, 5 U.S.C. 522, as amended, the Coalition for Safe Power requests the fol-lowing documents regarding the k'ashington Public Power Sup-ply System:

1.

Provide all documents relating to the accidental death of workers at h*NP-1-2-3-4-5 2.

Provide all documents concerning foreign material buried in the WNP-3 containment structure.

3. Provide all NRC documents concerning the evaluation of hTPSS management.
4. Provide all NRC documents evaluating the ability of WPPSS Quality Assurance Program at WNP-1-2-3-4-5
5. Provide all documents relating to the granting of ex-tensions of Construction Permits for WNP-1-2-3-4-5
6. Provide all documents concerning the March 8, 1981 ac-cident at WNP-5 including all analyses of the failure of the steel formwork and subsequent actions taken by the NRC on this incident.
7. Provide all documents related to worker comp aints, l

or submittals from workers on mismangement, safety prob-lems and construction practices at WNP-1-2-3-4-5

8. Provide all documents concerning an investigative com-mittee formed following two accidents at WNP-2 in August, 1977 on the part of WPPSS and all NRC documents related to safety practices and working conditions at WNP-2.

In our opinion, it is appropriate in this case for you to waive copyinc charges, pursuant to 5 U.S.C. 552 (a)(4)(A)
