Appendix NOTICE OF VIOLATION Calumet Testing Services, Inc.

License No. 13-16347-01 As a result of the inspection conducted on January 27, 1982, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),

the following violations were identified:

1.

License Condition No.17 of amendment No. 5 designates those individuals authorized to act as radiographers or radiography assistants as defined by 10 CFR 34.2.

Contrary to the above, two individuals not listed as either authorized radiographers or radiography assistants performed radiographic operations on August 13, 1981.

This is a Severity Level V violation (Supplement VII).

License Condition No.16 of amendment No. 5 states that the licensee shall possess and use licensed material in accordance with statements, representations and procedures listed. in application dated October 29, 1974..d of that application states that no person shall be per-mitted to perform radiography or be present in a controlled radiation area without a proper film badge.

Contrary to the above, on August 13, 1981, an individual entered a controlled area in which radiographic operations were being carried out without his personnel monitoring devices.

This is a Severity Level V violation (Supplement VII).

10 CFR 34.43(b) requires that physical radiation surveys be made after each radiation exposure during a radiographic operation to determine that the sealed source has been returned to its shielded condition.

This survey is to include the circumference of the camera and the guide tube.

Contrary to the above, on August 13, 1981, a radiographic operation had been completed and a complete purvey of the circumference of the camera and the guide tube was not per ormed.

This is a Severity Level V violation (Supplement VII).

l Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the_date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
