St. Elizabeth Medical Center FEB 4 1982 During this' inspection, our inspector noted that the teletherapy time' treatment chart for June,. July, and August 1981, was used to determine treatment times in December 1981.
In your response to this letter, please described your method to assure that the teletherapy treatment times are determined from the treatment chart for that month.
This inspection included a review of corrective actions taken in response to our inspection conducted on October 13, 1977. We have no further question in regard to these matters.
No items of noncompliance with NRC requirements were identified during the course of the inspection of License No. 12-14132-02 and License No. SNM-1875.
During the inspection of License No. 12-14132-03, certain of your activities appear to be in noncompliance with NRC requirements, as described in the enclosed Appendix. A written response, submitted under oath or affirmation, is required.
The responses directed-by this letter (and the accompanying Notice) are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.

e Appendix NOTICE OF VIOLATION Saint Elizabeth Medical Center License No. 12-14132-03 As a result of the inspection conducted on December 7 and 8, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
License Condition No. 18 states that prior to initiation of a treatment program, and subsequent to each installation of a teletherapy source, radiation surveys and tests shall be performed in accordance with 18A, B, and C.
License Condition No. 18C states that a report of the results of the surveys and tests described in 18A and B shall be sent to the Materials Branch, Division of Materials and Fuel Cycle Facility Licensing, U.S.
Nuclear Regulatory Commission, Washington, D.C. 20555, not later than thirty (30) days following each installation of a teletherapy source. A copy of such report shall be sent to the Director of the appropriate Nuclear Regulatory Commission Regional Office of Inspection and Enforcement.
Contrar to these equirements, you failed to submit a report of the results of the surveys and tests required by License Condition 18A and B, after receiving a new cobalt teletherapy source on August 10, 1979.

This is a Severity Level VI violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Consideration may be given to extending your response time for good cause sbown.
