This refers to the routine safety inspection conducted by Ms. E. Matson of this office on January 13 and 14, 1982, of activities authorized by NRC Byproduct Material Licenses No. 21-04082-01 and No. 21-04082-03 and to the discussion of our findings with Carolyn Meyer, Ross Bunting and others of your staff at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the g&

tively. Based on the evidence that the August badge was contaminated and that the September badge was water damaged we agree with your conclusion that the exposures were invalid and do not constitute actual exposures to the individual.
During this inspection, certain of your activities under License No.
21-04082-01 appeared to be in noncompliance with NRC requirements, as specified in the enclosed Appendix. A written response, submitted under oath or affirmation, is required.
No items of noncompliance were identified for License No. 21-04082-03.

Detroit Osteopathic Hospital l Corporation FEB 2 - 1982 We have noted that during 1981 five diagnostic misadministrations have occurred at Detroit Osteopathic Hospital Corporation. While misadmin-istrations are not in themselves violations of NRC regulations we are concerned that the number may indicate a weakness in safety practices or training. Therefore, in your response to this letter please indi-cate what actions you will take to prevent future errors in dose administrations.
The responses directed by this letter (and the accompanying Notice) are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
We will gladly discuss any questions you have concerning this inspection.
